- This is a combined synopsis/solicitation for supplies prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Reference number F6THCC9209A002 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-25. This solicitation is set aside for small business; the associated NAICS code is 334517 and small business size standard is 500 employees. This requirement is for the purchase of: Item Description 0001 - UVGI Fixture: V-Ray FO Normal Output by UltraViolet Devices, Inc. (www.uvdi.com) or approved equal, 61 inches long, 120V/220V, mountable in any orientation with permanently fixed mounting clips, complete with ballast & lamp. ETL listed, ABQK catagory. ETL listed under ABQK product category 24 EACH 0002 - Discharge Duct Lamp: V-Strike Normal Output UVC System by UltraViolet Devices, Inc (www.uvdi.com) or approved equal, 32.5 inches long for up to 10 ton systems, installed from outside ductwork via a single hole for the lamp, side or top mount installation, switchable 120/240V power, terminal block for power & safety interlock connections, complete with ballast & 2 lamps. UL/C listed under ABQK category 1 EACH 0003 - Ultraviolet Lamps, 61 inches long, P/N 07-4006 by UltraViolet Devices, Inc. (www.uvdi.com) or approved Equal. 4 EACH 0004 - Access/View Port Door (8 inches x8 inches ) by UltraViolet Devices, Inc. (www.uvdi.com) or approved equal. Certified by the UltraViolet Lamp manufacturer to protect from all ultraviolet transmissions from the lamps. 6 EACH Delivery requirement: 30 days ARO, FOB Destination at Mcchord, WA. 98430 Exact address will be provided upon award.
