Description
1. SCOPE OF WORK. The scope of work under these specifications consists of furnishing all labor, equipment, supplies and supervision necessary for the performance of all operations incidental to the installation of a new concrete sidewalk and pad as shown on the attached drawings and installation of government furnished playground equipment and impact tile on site. 1.1. Installation shall be for a surface mount Miracle model 714-S150 playground. After constructing and securing the playground equipment to the pad, impact tiles are to be glued in place on the surface of the concrete. All playground equipment, rubber impact tiles and tile glue are furnished by the Government for this contract. 1.2. Installation of this playground equipment shall be completed by July 1, 2010. 2. MEETING WITH THE CONTRACTING OFFICERS REPRESENTATIVE (COR). 2.1. The Contractor shall meet with the COR prior to commencement of work to discuss performance requirements, work schedule and administration of the contract. The Contractor shall also supply a safety plan at that time for the approval of the COR. The Contractor shall designate an on the job supervisor to act on the Contractors behalf when he or she is absent. 3. CONTRACTOR REQUIREMENTS. 3.1. The Contractor shall be a certified Miracle installer and have sufficient knowledge and skills to install this equipment as specified by the manufacturer. 3.2. The Contractor shall provide all necessary materials not supplied by the Government to install the structure. The Contractor will be responsible for securing the area during construction. C-PAGE-1 3.3. The Contractor will keep the area off limits to the public by adequate fencing or other control structure until all work is completed. The COR will perform a final inspection after notification by The Contractor that the project is finished. When the COR finds that the equipment is properly installed and ready for public use, the Contractor may remove any barricades and allow public access. 4. ACCIDENT PREVENTION. 4.1. The Contractor shall comply with all pertinent provisions of Corps of Engineers manual, EM-385-1-1, entitled Safety and Health Requirements, as amended, available from the Lake Manager, Gavins Point Project, Yankton, South Dakota. 4.2. The Contractor shall comply with applicable Occupational Safety and Health Act (OSHA) Standards. 4.3. The Contractor will maintain an accurate record of all accidents resulting in death, traumatic injury, occupational disease, and damage to property, materials, supplies and equipment incident to work performed under this contract. Accidents shall be reported to the COR within 12 hours. 4.4. Compliance with the provisions of this article by subcontractors will be the responsibility of the Contractor. 5. SPECIFICATION FOR CONCRETE. 5.1. Concrete and base material for the sidewalk and slab for the playground equipment shall meet the requirements of the South Dakota Department of Transportations Standard Specifications For Roads and Bridges, Section 651, Concrete Sidewalk. Concrete shall also be fiber reinforced. The joint between the concrete slab and the sidewalk will be dowel reinforced to prevent settlement. 6. COORDINATION WITH OTHERS. Other contractors or government personnel will be working in assigned areas during this contract. The Contractor shall make every effort to work in harmony with all involved parties.
