Description
STATEMENT OF WORK The U.S. Naval Hospital Guam has a requirement for repairs and paintings of all interior ceramic wall tiles to include existing concrete surface and toilet partitions with new water based tile-clad epoxy finish for the Hospital. This is a combined synopsis/solicitation for commercial items prepared in accordance with the FAR Part 12 and FAR Part 13 simplified acquisition procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. The solicitation number is N6809609RC90172 and is being issued as a Request for Quote (RFQ). The North American Industry Classification System (NAICS) Code is 238990. The Government anticipates award of a firm fixed-price order. The contractor shall provide services in accordance with the attached Statement of Work. The contractor shall visit the job site. All prospective offerors shall inspect the site where the services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the price of contract performance to the extent that the information is reasonably obtainable. Contractor shall provide all labor, materials, equipment, supervision and incidental related work necessary for the repairs and painting of all restroom ceramic wall tiles to include the existing concrete wall surface. A site visit is scheduled for 16 Sept 2009 at 9:30 a.m. Meet at room L100B, Facilities Management Conference Room first floor for preliminary discussions and instructions. The following FAR Part 12.3, and FAR Part 13.0 with provisions and clauses are applicable to this acquisition: 52.212-1 (June 2008) Instruction to Offerors-Commercial Items; 52.212-2 (Jan 1999) Evaluation-Commercial Items; offerors are required to provide a complete copy of the provision at 52.212-3 (Feb 2009) Offeror Representations and Certifications-Commercial Items; 52.212-4 (March 2009) Contract Terms and Conditions-Commercial Items; 52.212-5 (May 2009) Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items. For the purpose of 52.212-2, evaluation will be based in order of importance: (1) Technically acceptable, (2) Past performance: The offeror shall demonstrate his past performance experience and provide any client reference information, (3) Price. All sources may submit a quotation which will be evaluated. Offerors may submit their quote in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.) but must include the following information: breakdown of costs (labor, material, manhours), complete mailing address, discounts for prompt payment if any, delivery/availability of products, DUNS number, and the Taxpayer Identification Number (TIN). Responses must be submitted electronically to Sue.Larrew@med.navy.mil or fax 671 344-9256. All respondents will be considered by this agency. The Government intends to make a single award to the responsive, responsible offeror whose quote, conforming to this synopsis/solicitation, will be the best value to the Government. All offers must be received by COB 4:30 p.m. local pacific time, 23 Sep 2009. No phone calls will be accepted. Contractor must be CCR registered.
