- This is a combined synopsis/solicitation for commercial itemsprepared in accordance with the format in Subpart 12.6, as supplementedwith additional information included in this notice. This announcementconstitutes the only solicitation; proposals are being requested and awritten solicitation will not be issued. Solicitation VA-246-09-RQ-0220that the solicitation is issued as an request for quotation.Thesolicitation document and incorporated provisions and clauses are thosein effect through Federal Acquisition Circular _36____. Notice to Service Disabled Veteran Owned Small Business (SDVOSB)Set-a-side. The contract for healthcare for homeless Veterans housingcontract. The contractor shall furnish transitional housing for thebeneficiaries of the Department of Veterans Affairs Medical Center(VAMC), Hampton, Virginia, 100 Emancipation Dr. Temporary ResidentialShelter provide housing for at least (4) veterans, to include, but notnecessarily limited to, beds, restroom, bath, and kitchen facilities,and related furnishings. The contractor shall provide services tobeneficiaries for whom such care is specifically authorized by theDepartment of Veterans Affairs Medical Center herein after referred toas the (VA). The contractor shall provide to the VA, Community BasedHealth Care for Homeless Veterans (HCHV).Provision at 52.212-1, Instructions to Offerors-Commercial, applies tothis acquisition and a statement regarding any addenda to the provision. Applicability of the provision at 52.212-2, Evaluation-Commercial Items,if used, and the specific evaluation criteria to be included inparagraph (a) of that provision. If this provision is not used, describethe evaluation procedures to be used. Offerors to include a completed copy of the provision at 52.212-3,Offeror Representations and Certifications-Commercial Items, with itsoffer. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items,applies to this acquisition and a statement regarding any addenda to theclause. Clause at 52.212-5, Contract Terms and Conditions Required To ImplementStatutes or Executive Orders-Commercial Items, applies to thisacquisition and a statement regarding which, if any, of the additionalFAR clauses cited in the clause are applicable to the acquisition. Any additional contract requirement(s) or terms and conditions (such ascontract financing arrangements or warranty requirements) determined bythe contracting officer to be necessary for this acquisition andconsistent with customary commercial practices. Proposal package due September 18, 2009. All correspondence andresponses shall be e-mailed only to Nakaura.yusuf@va.gov. Because thesynopsis and solicitation are contained in a single document, it is notnecessary to publicize a separate synopsis 15 days before the issuanceof the solicitation.
