Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36 as corrected. This solicitation is set aside for Small Businesses. The NAICS code is 813920, with a size standard of $7.0 million. The Contractor will provide these services at the VA Central California Health Care System in Fresno, CA. The contract performance period will be for one year starting on or about October 1, 2009 through September 30, 2010, with a one-year option. Electronic, hand-delivered or properly mailed offers will be accepted. Faxed offers will not be accepted. Offers are due to eric.toliver@va.gov by NLT 3:00 p.m. on September 18, 2009. Offers must include a completed price schedule, curriculum vitae for each pharmacist, and a maximum of three contract references (with correct contact information). Price must include an hourly rate for on-call call backs. Curriculum vitae will be used for technical evaluation based on skill sets and experience. Contract references will be used for past performance and must be within the last three years (September 16, 2006). For information regarding the solicitation you can e-mail me at the above e-mail address or phone at (916) 923-4528. The Statement of Work and Price Schedule are as follows: 1. Scope of Work 1.1. The Contractor shall provide Pharmacist services to the VA Central California Health Care System (VACCHCS) located at 2615 E. Clinton Ave, Fresno, CA for the period October 1, 2009 through September 30, 2010. Pharmacist services shall be provided on an as-needed basis in accordance with the terms and conditions of this contract at the hourly rates specified in the Price Schedule. The Contractor's price includes all necessary travel time, mileage charges and per diem to and from the Contractor's place of business. 1.2. Pharmacy services include the interpretation of physician orders, dispensing of appropriate medications, and counseling patients on medication administration and storage. The overall duties require a comprehensive knowledge in current clinical practice in patient drug therapy emphasizing geriatric and rehabilitation medications and delivery systems. The Contractor will, when assigned, rotate through each operational area and/or shift of the Pharmacy Service. The Contractor is expected to comply with all mandated standards and requirements. 2. Specific Contractor Tasks: 2.1.Clinical: 2.1.1. Provides accurate information for all inquiries from patients, family members, health professional, students, etc. Will provide follow through and resolution to an acceptable end. 2.1.2. Effectively counsels a patient on drug interactions, adverse side effects, food-drug interactions, and compliance issues. 2.1.3. Provides consultations on proper drug regimens, contraindications, drug interactions, product availability and pharmacy policies/procedures, Monitors and assesses various prescribed therapies (e.g., aminoglycoside antibiotics, antipsychotics, digoxin, theophylline, etc.) using pharmacokinetic techniques and will assist Clinical Specialists in monitoring their assigned areas. 2.2.Distributive: 2.2.1.Evaluates patients' medication orders and dispenses appropriate medications and dosage forms, properly labeled after reviewing for completeness, eligibility and appropriateness of drug, dose, route, strength, and quantity to be dispensed. Reviews for drug incompatibilities, inappropriate combinations, potential adverse drug reactions, dosage ranges, intra-agency polypharmacy practices and initiates cancellation of non-active or duplicate prescriptions. Inputs appropriate information into the VA computer system (Outpatient, Unit Dose, IV Admixture). 2.3.Administrative: 2.3.1. Ensures the Chief of Staff, the Chief Pharmacy Service, or their designee is informed of drug inventory which includes monitoring of effective stock levels, expiration dates, proper storage conditions, and inventory control procedures. 2.3.2. Prioritize work, adjust dispensing/preparation activities, and accomplishes effective clinical and distributive services. 2.3.3. When assigned, performs other duties that are deemed essential to the pharmacy operation. 2.4. Normal Duty Hours: VACCHCS normal duty hours are 7:30 am to 4:30 pm Monday through Friday. The Contractor will provide service on an as required basis as scheduled by the VA Chief, Pharmacy Services. Typically Contractor hours will be Monday through Friday from 9:30 am to 6:00 pm or 8:00 am to 4:30 pm. However there may be occasion when the Contractor will required to work outside these hours. The contractor will not be required, except in case of emergency, to furnish such services on a national holiday. A Contractor's employee must be present at the VA facility and must be actually performing the required services for the period specified in the contract. 2.6. A record keeping system of Contractor hours shall be established. The payments for any leave, including sick leave or vacation time, are the responsibility of the Contractor. 2.7. The contractor's personnel shall agree to comply with the Privacy Act of 1974 (the Act) and the VACCHCS rules and regulations issued under the Act during and after execution of this contract. Any information about an individual that is maintained by the VA, including, but not limited to education, financial transactions, medical history, and criminal or employment history or any record that contains the person's name, or the identifying number, symbol, or other identifying particular assigned to the individual, shall be kept CONFIDENTIAL. 2.8. The Contractor Pharmacist shall work and communicate effectively while remaining courteous, tactful, and understanding when interacting with others, even in potentially difficult or volatile situations. 3. Qualifications: 3.1. Contractor shall provide the services of a fully trained registered and licensed Pharmacist. 3.2. Perform the services covered by this contact, the Contractor's personnel shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Qualifications shall be subject to the review by the VA Chief of Staff and approval by the VA facility Director. 3.3. The Contractor's personnel shall be responsible and able to show evidence of reliability, ability and experience. Verified references of ability and qualifications may be accepted by the Contracting Officer to satisfy these requirements. 4. Payments The Contracting Officer's Technical representative (COTR), in conjunction with the Contractor, shall keep accurate records of hours worked. Based on subject records, the Contractor shall prepare proper documents for payment for services rendered. Payment shall be made monthly in arrears upon receipt of a proper invoice from the Contractor. 5. VA provided materials/services: The VA will provide the Contractor's employee with all pharmaceutical supplies and facilities, including VA operating instructions/regulations, without limitation; uniforms where applicable and necessary to enable the pharmacist to perform each and every confirmed work assignment in a clean, safe, and professionally satisfactory manner. Under no circumstances will the pharmacist supplied under this contract be responsible for failure of performance due in any way to lack of supplies or facilities. 6. Changes after award The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contractor administration. Only the Contracting Officer is authorized to make commitments or issue changes that will affect price, quantity, or quality of performance of this Agreement. ln the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to be made without authority and no adjustment will be made in the contract price, terms, or conditions to cover any increase in costs incurred as a result thereof. 7. Status of Independent Contractor The relationship between the VACCHCS and the Contractor's personnel is not and shall not be construed or interpreted to be a partnership, joint venture or agency. The relationship between the VACCHCS and the Contractor's personnel is an independent contractor relationship.
