Sources Sought Document Type: Sources Sought Solicitation Number: N/A Posted Date: 4 September 2009 Response Date: 6 October 2009 This is a sources sought announcement only! This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of potential qualified sources and their business classification, such as service disabled veteran owned business (SDVOB), 8a, HubZone, small disadvantage, etc, relative to NAICS 621111, size standard $10 Million. Responses will be used to determine appropriate acquisition decisions. The Department of Veterans Affairs, William Jennings Bryan Dorn Veterans Affairs Medical Center (VAMC), 6439 Garners Ferry Road, Columbia, SC 29209 is seeking sources for a potential contract to provide the establishment of a Community Based Outpatient Clinic (CBOC) in Rock Hill, SC. The contractor is to provide the continuous delivery and management of primary care and mental health for all assigned Veteran Affairs (VA) patients at their establishment. The Contractor will provide health care providers, medical facilities, medical equipment and supplies, and all administrative functions to fulfill the care and range of services as required by the VA and Joint Commission on Accreditation of Healthcare Organizations (JCAHO), or other regulatory agencies. (NOTE: Contractors facility must be physically located within Rock Hill zip code 29732). The Contractor is responsible for ensuring that all persons, whether they be employed, agents, sub-contractors, providers, or anyone acting for or on behalf of the contractor, are properly licensed at all times under South Carolina state law and/or regulations and shall be subject to credential and privilege approvals by the VA. The intended contract is anticipated as one base year, with four (4)-one year option periods. NOTE: The WJBD VAMC Columbia will be scheduling an internal audit/study to determine the feasibility or possibility of changing from a Contracted facility to a VA Staff facility within the next 2-3 years. Qualified offerors shall be experienced in providing the continuous delivery and management of primary care and mental health for all assigned Veteran Affairs (VA) patients and all administrative functions to fulfill the care and range of services as required by the VA and Joint Commission on Accreditation of Healthcare Organizations (JCAHO), or other regulatory agencies. It is anticipated that a Request for Proposals will be issue sometime during the first quarter of FY10 with an anticipated contract award date during the latter part of FY10 with a 120 day start-up period from date of award. Please submit expressions of interest in your own format with the minimal information as specified below in sufficient detail for the Government to determine that your company possesses the necessary expertise and experience to compete for this acquisition.
