- RETURN PACKET Solicitation # R6-15-09-302641 THIS COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a complete package is attached for offerors to return. This notice constitutes as Solicitation R6-15-09-302641 and is being issued as a Request for Quotation. This notice and any incorporated provisions include those in effect through Federal Acquisition Circular 2005-27. Quotes are being solicited from Small Business Concerns only. The NAICS code associated with this acquisition is 324121 and the small business size standard for that code is 500 employees. DESCRIPTION: Supply Asphalt Concrete Patching Mix for the Umpqua National Forest, Diamond Lake District, 2020 Toketee Ranger Station Road, Idleyld Park, OR 97447. The Contractor’s plant shall be located in Roseburg, OR to allow for the US Forest Service dump trucks to transport the hot asphalt mix to Forest Service managed roads within an 80-mile radius of Roseburg, OR. PERIOD OF PERFORMANCE: The anticipated contract time is 60 days, for a period of time between September 8, 2009 through September 30, 2010, weather dependent. PROVISIONS AND CLAUSES: The following provisions and clauses are hereby incorporated into this RFQ by reference; (full text of the clauses may be accessed electronically at www.arnet.gov/far/): FAR 52.212-1, Instructions to Offerors-Commercial Items (June 2008) and FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Oct 2008). The following provisions and clauses are hereby incorporated into this RFQ in full text in the attachment “09-302641 Solicitation.doc” : FAR 52.212-2, Evaluation- Commercial Items (Jan 1999), FAR 52.212-3, Offerors Representations and Certifications-Commercial Items (Jun 2008) and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items (Oct 2008) QUOTE DUE DATE: Quotes shall be received by Wednesday, September 2, 2009, at 4 p.m. PST.
