Description
This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. The request for this number is N6847009RN001. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items; additional FAR clauses cited in the clauses are: 52.222-3, Convict Labor; 52.222-26, Equal Opportunity: 252.232-7003, Electric Submission of Payment Requests and Receiving Reports, NAICS for this requirement is 339112. Intension is to award a Firm-Fixed Price contract. The U.S. Naval Hospital, Okinawa, Japan intends to negotiate a sole-source contract with JEMED for the following items: Pediatric ENT-2L Machida Scope, 2.2mm, Q’ty: 2ea, Standard ENT-3L Machida Scope, 3.0mm,Q’ty:2ea, Standard ENT-4L Machida Scope,4.0mm, Q’ty: 2ea. 4. The contractor shall be responsible for direct delivery to Physical address of US Naval Hospital, Okinawa Japan BLDG 6000, Camp Lester 904-0103, via Naha Airport. The contractor shall also be responsible for delivery to Tracy, CA 45-50 days ARO and providing proof of delivery and tracking information (tracking number) shall be provided to the Government upon request. Contractors shall re-ship orders, at their expense, if proof of delivery and proper tracking information cannot be provided. This notice of intent is not a request for competitive quotes; however, any firm that believes it can meet the requirements may give written notification to the Contracting Officer within 10 calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail, to demonstrate the contractor’s capability to provide and ability to comply with the above requirements. Information can be e-mailed to the Contracting Officer. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price; all to be submitted with quotation. No telephone requests will be accepted. Responses received will be evaluated, however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the U.S. Naval Hospital, Okinawa, Japan will proceed with the sole source procurement with MSR Northwest Inc. Faxed offers will be accepted. Offerors are responsible for obtaining any and all amendments or additional information. Quotes and all pertinent data (descriptive literature, specifications, etc.) must be submitted to the POC no later than September 9, 2009 Point of contact is Fumikazu Higa e-mail: fumikazu.higa@med.navy.mill).
