Description
THIS NOTICE IS PROVIDED FOR INFORMATION PURPOSES ONLY. THIS OPPORTUNITY IS AVAILABLE ONLY TO CONTRACTORS UNDER W912C3-09-T-0034. To include revised Performance Work Statement as follows: 1. SCOPE OF WORK. The contractor shall serve as the Roman Catholic Priest for the Fort Buchanan Catholic population providing services as a priest working in accordance with the Catholic religious program. Work shall be accomplished in accordance with the requirements set forth in this performance work statement. The services shall include the following: 1. Provision of Roman Catholic worship services, religious counsel, and scripture study sessions and religious education. 2. Pastoral care within the military setting for eligible recipients to include marriage preparation, crisis response and intervention, personal and family pastoral counseling and coaching and hospital visitation to the sick /injures. 3. Advice the command leadership regarding ethics, moral and morale. 4. Training for leaders in ethics morals decision making and religion culture 5. Assist the Command chaplain in the facilitation of religious requirements for all faiths as a member of the Religious Ministry Team. Tasks: The Roman Catholic Priest Chaplain Worship services shall include but not limited to the following: 1.HOURS OF OPERATION. The contractor shall provide the required products and services as indicated in this Performance Based Statement (PWS) and in the delivery schedule of the contract. Weekend Mass will be conducted on Saturdays 1700-1800 hrs, Sundays at 1300-1400 hrs. Service schedule is not subject to changes. Perform Catholic Holy Mass and confessions for active duty personnel and all other authorized personnel as defined in accordance with Catholic Priest Manual and AR165-1, Religious Support in the US Army. 1.1 Conduct Catholic Holy Mass every Sunday and Saturday 1.2 Conduct Weekday Holy Mass as required 1.3 Conduct Catholic Confession every Sunday one hour prior to the start of Holy Mass and one half hour after the end of each Mass. 1.4 Conduct Catholic Masson all Holy Days of Obligation 1.5 Conduct Advent and Lenten Penance Services 1.6 Conduct First Communion and Confirmation services as required. 1.7 (Deleted) 2. Perform Sacramental and emergency ministrations for Active Duty personnel and family members including Baptism, Funeral and Memorial Services and Anointing of the Sick. 3. Services provided during week day working hours shall include but are not limited to: 3.1 Sacramental record Preparation 3.2 Pastoral Care, counseling, and spiritual support for Catholic Preferences 3.3 Inform the Command Chaplain 3.4 Monitoring the Catholic religious education program 3.5 Various administrative requirements 3.6 Deleted 3.7 First Communion, Confirmation Preparation 3.8 Maintain, First Communion, Confirmation Preparation 3.9 Maintain and publish a weekly bulletin 3.10 Ensure compliance with all financial regulation with regards of the Chaplain Tithes Offering Funds (CTOF) and Standard Operating Procedure (SOP). 3.11 Compose, prayers of the faithful, for Sunday and Holy Day Liturgies with respect to Liturgical Guidelines and the needs of the Command Master Religious Program for the Installation. 4. Submit problems, concerns, comments and recommendation, particularly for improvement regarding Catholic Services of the Catholic program needs to the Command Chaplain. 4.1 Lessons Learned shall be provided in written to the Command Chaplain within 5 days of any problems encountered and resolved. 4.2 Concerns regarding morale shall be directed to the command Chaplain as soon as possible. 5. Notify the Command Chaplain when unable to perform services due to scheduled time off, illness, inclement weather and or emergency leave. 5.1 Arranged for scheduled absences at least two weeks in advance in written. 6. Comply with station and government policies and regulations in regard to identification badges, parking, and professional decorum. 7. Deliverables. All deliverables shall be delivered to the Primary Technical point of contact identified in paragraph 7. 8. PLACE OF PERFORMANCE. The contractor shall perform services at the Chaplains Office spaces and other places to be determined. 9. PERSONNNEL QUALIFICATIONS. Offeror must be an Ordained Roman Catholic Priest in good standing and endorsed by the Archdiocese for the Military Services, USA. Personnel proposed for this contract shall have the requisite skills and experience consistent with Roman Catholic Priests whose background and experience are acceptable to the Archdiocese for the Military Services, USA. Personnel proposed for this contract must be able to qualify for a Confidential Security Clearance at a minimum. Qualifications: The contractor shall meet the following qualifications: (a) Shall have received the Sacrament of Holy Orders in the Roman Catholic Church. (b) Shall be qualified to perform all elements of the contract outlined in the Scope of Work. Specifically, the contractor, before accepting the contract, must provide the Archdiocese For the Military Services, USA (AMS), P.O. Box 4469, Washington, D.C. 20017, 202-269-9022. (c) A letter of permission from his Bishop or religious superior, or the Archbishop of the Archdiocese for Military Services, stating that he is a priest in good standing and that he has permission of the Bishop/religious superior to accept a contract. (d) Submit an application for endorsement by the AMS, 9.1. Offerors must have a clear understanding of ARMY organization and the pluralistic ministry requirements that affect the use of Chapel facilities and staff. 9.2. Not Applicable. Already Addressed. 9.3. Offerors must provide a Certificate of Ecclesiastical Endorsement. 9.4. Offerors must possess a general working knowledge of Microsoft Word, Microsoft Excel, and Microsoft Outlook or equivalent software and desktop computer and hardware. 9.5. Offerors must demonstrate reliability, ability to interact with people from all socio-economic backgrounds whether military or civilian (including family members and retirees), as well as good oral and written communication skills. 9.6. Offerors must have a positive personality and pleasant demeanor when interfacing with chapel staff, visitors, military personnel, parishioners, and others. 9.7. Offerors must be able to perform primarily sedentary work. Some walking, standing, carrying of light items, cleaning of immediate work area may be required. 9.8. The offeror shall be neat, clean, well groomed, and in appropriate clergy clothing when in public areas. The contractor shall display the identification badge provided by security any time while at work. 9.9. The offeror shall become acquainted with and obey all pertinent US Army, and DOD regulations, shall perform in a manner which precludes any waste of utilities and shall not use government equipment for personal business. All motor vehicles operated on this installation by the contractor shall be registered with base security and operated according to applicable directives. Smoking by the contractor is restricted to designated areas. 9.10. The offeror, although in fact not a Government Employee, shall comply with Executive Order 11222 of May 8, 1965, Prescribing Standards of Ethical Conduct for Government Officers and Employees 10. Points of contact Designation of Technical Representative: The Fort Buchanan Garrison Chaplain, Chaplain (LTC) Kenneth E. Lawson, 787-707-3904, or his authorized representative is designated as the Technical Representative of the Contracting Officer for the purpose of technical surveillance of performances and inspection of materials for work being performed under this contract. The Fort Buchanan Garrison Chaplain will monitor contractors performance to ensure the contractor possesses the required faculties and qualification. No inspector is authorized to change any provision of the specifications; nor shall the presence or absence of an inspector relieve the contractor from any requirements of the contract. c. Receiving report responsibility: The Fort Buchanan Garrison Chaplain is responsible for preparing the receiving report and ensuring that the contractor is paid monthly for the amount of services performed. The contractor is solely responsible for the monthly submission of invoices of services rendered, via Wide Area Work Flow (WAWF) (instructions attached). Invoices must be submitted the following duty day after the last Sunday of each month. 11. CONTRACTOR PERSONNEL a. Competency of Contractor Personnel. All services under this contract are to be performed by an adult with verifiable moral character; spiritual maturity and credentials authorized by the Catholic Church and verified by the Garrison Chaplain via AMS. Applicants must be willing to undergo a background check. The contractor shall be fully qualified to perform the required services without any supervision or direction by government personnel. b. Contractor Points of Contact. The contractor shall provide the Contracting Officer and the COR with the names of a primary and alternate point of contact with telephone numbers, in case of emergency. c. Personal Property. The contractor shall ensure that personal property is clearly marked and identified to preclude misidentification as government property. 3. (Deleted) 12. GOVERNMENT-FURNISHED PROPERTY AND UTILIITES a. Office and equipment. The government will furnish office space at the Religious Support Office and/or Garrison Chapel, with appropriate office furnishings and equipment, for the support of the contractor only in the actual provision of products and performance of services. Said office is not permanent or dedicated solely to the use of the contractor. b. Appropriate office furniture and equipment includes a desk, chair, filing cabinet and, subject to availability, copier services, audio-visual equipment, curriculum, and standard liturgical supplies. c. UTILITIES. The government will provide all utilities, such as electrical and water, to perform the services required by the contract, at the site of delivery of the services on federal property. The government will not make any modifications to utility outlets to accommodate contractors equipment. d. The government shall not furnish the contractor with government living quarters. 13. CONTRACTOR-FURNISHED PROPERTY AND UTILITIES a. The contractor shall furnish his/her own transportation to and from the work site(s). b. The government will not be liable for loss, damage, theft, or maintenance of personal equipment. 14. CONTRACTOR RESPONSIBILITIES a. The contractor is solely responsible for all tax liabilities. The contractor is required to provide Workers Compensation coverage via an insurance document prior to commencement of any services. This is a Non-Personal contract. b. The contractor shall provide a qualified substitute in the event the contractor cannot meet any obligation of this contract. A qualified substitute is defined as an individual who meets the qualifications outlined in paragraphs 2.a.b.c of this contract, Competency of Contract Personnel. Any substitute must meet the approval of the Garrison Chaplain and the AMS. c. The contractor shall regard personal information on all applicants, volunteers, and participants as confidential. As required by public laws and military regulations, the contractor will divulge information of illegal activities to the proper authorities. d. The contractor shall present a neat, well-groomed appearance appropriate for chapel worship services during all times while he is performing services under this contract. 15. ADMINISTRATION. Administration for the purpose of this contract is the completion of reports, government forms, and other input that may be required for each contract product service. a. Document of Work/Reports. The contractor shall maintain a log of the work performed under this contract, and shall provide a copy of the log to the Garrison Chaplain on a monthly basis. b. Deleted. c. Request for Supplies/Equipment. The contractor shall provide the Garrison Chaplain with the proper requests for the ordering of supplies and equipment to support the Catholic Program in accordance with the Consolidated Chaplains Fund SOP and the Resource Managers instructions (for appropriated fund supplies). The Chaplain Resource Manager will provide applicable forms, SOPs, and guidance to the contractor. d. Privacy Act. The contractor shall be familiar with and ensure compliance with provisions of the Privacy Act, Freedom of information Act, and limitations on government collection of personal and statistical data on non-affiliated personnel. Applicable government regulations concerning these acts will be made available to the contractor for review by the Staff Chaplain. (End of Summary of Changes)
