Attachment 4, General Information Attachment 3, U.S. Department of Labor Wage Determination No. 2005-2043, Rev. 10 Attachment 2, Performance-Based Work Statement Attachment 1, Supplies/Services - Shop Towels Total Small Business Set-Aside. Solicitation FA9302-09-R-0028 is a Combined Synopsis/Solicitation for commercial items prepared IAW the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This is a request for quotation (RFQ). Quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-35 and DFARs Change Notice (DCN) 20090729. NAICS is 812320 and the small size standard is $4,500,000.00 (average annual receipts). The AFFTC/PKE, Edwards Air Force Base, CA is seeking sources capable of providing the following services: Shop Towel Services per attached Supplies/Services - Shop Towels; Performance-Based Work Statement; Wage Determination 2005-2043, Rev. 10; and General Information. The period of performance is as follows: Item 1: 0001AA through 0001AK, Year One (1), 1 Oct 09 - 30 Sep 10 Item 2: 0002AA through 0002AK, Year Two (2), 1 Oct 10 - 30 Sep 11 Item 3: 0003AA through 0003AK, Year Three (3), 1 Oct 11 - 30 Sep 12 Item 4: 0004AA through 0004AK, Year Four (4), 1 Oct 12 - 30 Sep 13 Interested parties who believe they can meet all the requirements for the supplies/services described in this synopsis/solicitation are invited to submit in writing a complete quote including required documentation referenced below. Offerors are required to submit with their quote enough information for the Government to evaluate the minimum requirements detailed in this synopsis.
