Description

FDA Current Title Listing Description of Requirements This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal and is identified as FDA-09-223-SOL-00151. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-36. This requirement is a full and open competition. The associated NAICS code is 424920 with a size standard of 500 employees. It is anticipated that the award will be made for a one (1) year base year and four (4) one year option contract periods. Prospective offerors are responsible for downloading the solicitation and any amendments. It is the offeror's responsibility to monitor the FedBizOpps website for the release of any amendments to this solicitation. DESCRIPTION OF REQUIREMENTS: see attached Solicitation FDA 09-223-SOL-00151 PERIOD OF PERFORMANCE: one base year and four (4) one year option Base period: October 18, 2009 through October 17, 2010. PAYMENT: 30 days after receipt of a proper invoice. CCR: Vendors must be registered in the Central Contractor Register (CCR) prior to the award of a contract. You may register by going to www.ccr.gov. You will need your Dun & Bradstreet number and banking information. QUESTIONS DEADLINE: All questions are to be submitted via email to Zetherine.gore@fda.hhs.gov no later than August 27, 2009, 2:00pm local time. PROPOSALS DUE: All proposals must be received electronically to Zetherine.gore@fda.hhs.gov and are due no later than 2:00pm, local time on September 9, 2009. There is a proposal page limitation of 25 pages. Proposals received from an offeror must not exceed 25 pages. ANTICIPATED AWARD DATE: The anticipated award date is September 18, 2009; however, all dates in this announcement are subject to change. PROVISIONS and CLAUSES: The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this solicitation. An addendum is attached to this provision. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items. The clause at FAR 52.212-4, Contract Terms and Conditions, Commercial Items applies to this acquisition. An addendum is attached to this provision. The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders, Commercial Items applies to this acquisition. The following FAR clauses cited are applicable: FAR 52.219-9, FAR 52.222-3, FAR 52.222.19, FAR 52.222-21, FAR 52.222-26, FAR 52.222-35, FAR 52.222-36, FAR 52.225-1, FAR 52.225-5, FAR 52.225-13, FAR 52.232-33.
