- THIS IS A SOURCES SOUGHT, HOWEVER IF YOUR COMPANY CAN PROVIDE THIS SERVICE, PLEASE PROVIDE A QUOTE FROM THE INFORMATION ON THIS RFI. THIS SERVICE IS FOR THE VA MEDICAL CENTER, FAYETTEVILLE, ARKANSAS 72703 Contract will begin October 1, 2009 through September 30, 2010 Statement of work for Medical Waste Removal Contract is for pick up, removal, and disposal of Medical Waste with complete documentation provided by the Vendor. Pick up Service every two weeks. ( Total of 26 stops per year) Vendor to provide 40 empty 28 gallon Medical Waste Containers. Vendor to pick-up 40 full 28 gal.Medical Waste Containers and twelve 20 gal full sharps containers for disposal. Service will be provided between the hours of 7:00A.M. to 3:30 P.M. Monday through Friday excluding Holidays. Vendor will assume full responsibility for disposal of waste.
