- Attachment 1 - Past Performance Questionnaire RFQ SAMHSA10356 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. A description of the requirement can be found in Section B, Scope of Work, in the attache document RFQ SAMHSA10356 along with other, detailed information. This request for quotation (RFQ) and incorporated provisions and clauses are those in effected through Federal Acquisition Circular 2005-36 effecitve August 11, 2009. This is a total small business set-aside under NAICS code 519130 with a size standard of 500 employees. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition there are no addenda to that provision. Firms shall include in their quotes a copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items.
