- THIS IS A SOURCES SOUGHT, HOWEVER IF YOUR COMPANY CAN PROVIDE THIS SERVICE, PLEASE PROVIDE A QUOTE FROM THE INFORMATION ON THIS RFI. THIS SERVICE IS FOR THE VA MEDICAL CENTER, FAYETTEVILLE, ARKANSAS 72703 CONTRACT WILL BEGIN ON OCTOBER 1, 2009 THROUGH SEPTEMBER 30, 2010. STATEMENT OF WORK: Filling of Government Owned Oxygen Cylinders Filling of government owned "E" cylinders with the mixture: 4% carbon dioxide, 16% oxygen, with a nitrogen balance. Fill approximately 6 cylinders per month. Filling of government owned "E" cylinders with oxygen. Fill approximately 95 cylinders per month. Filling of government owned "H" cylinders with oxygen. Fill approximately 12 cylinders per month. Filling of government owned "H" cylinders with nitrous oxide. Fillapproximately 4 cylinders per month. Filling of government owned "E" cylinders with nitrous oxide. Fillapproximately 2 cylinders per month. Filling of government owned " E" cylinders with nitrogen, medical grade, nonflammable. Fill approximately 6 cylinders per month. Filling of government owned" E" cylinders with carbon dioxide. Fillapproximately 10 cylinders per month. Filling of government owned 20 pound cylinders with carbon dioxide. Fillapproximately 6 cylinders per month. Filling of government owned pulmonary diffusion gas consisting of 0.3% carbon monoxide, 10% helium, 21% oxygen with a nitrogen balance. Fillapproximately 4 cylinders per month.Filling of government owned "H" cylinders with compressed air. Fillapproximately 10 cylinders per month. Filling of government owned "H" cylinders with breathing air. Fillapproximately 5 cylinders per month. Filling of government owned "E" cylinders with 26% oxygen with a nitrogenbalance. Fill approximately 6 cylinders per month. Filling of government owned 25 liter liquid nitrogen. Fill approximately 2cylinders per month. Filling of contractor owned" H" cylinder with "dry nitrogen" for use in AC. plant. Fill approximately 6 cylinders per year. Filling of contractor owned 20 pound cylinder with nitrous oxide. Fillapproximately 2 cylinders per month. Filling of contractor owned "H" cylinder with carbon dioxide (not required to be medical grade- for use in laboratory). Fill approximately 2 cylinders per year. Filling of contractor owned "H" cylinders with an anaerobic growth mixtureconsisting of 5% carbon dioxide, 5% hydrogen, with a nitrogen balance. Fill approximately 2 cylinders per month. Monthly rental of three MRI Compatible Walk about Tanks. Deliveries: Contractor will be required to deliver 2 times per week on a scheduled basis and more if an emergent need exists. Deliveries will be made between hours on Monday thru Friday between 8 am. and 4:30 p.m. Upon arrival at the Medical Center the Contractor will call extension 5353 for access to the oxygen room. Contractor will not require either a key or badge for deliveries
