- SF18, SOW, EVALUATION AND CLAUSES This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is being issued using simplified acquisition procedures 13.5. This solicitation is a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-34. This solicitation is a 100% Small Business Aside. The associated North American Industrial Classification Systems (NAICS) code for this procurement is 621330, Offices of Mental Health Practitioners. Offerors must submit Quote in response to solicitation CSSOA-09-Q-0024 prior to August 12, 2009 at 3:00 P.M. EST (local). Electronic versions of the offeror’s quote will not be accepted. Offerors may submit quote via overnight express, by courier, in person delivery or by U.S. Postal Services.
