Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for FA4800-09-JD-2005. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-12. This acquisition is 100% set-aside for small businesses. The associated NAICS code is 813110 with a $7,000,000.00 size standard. This RFQ contains 5 line items. The line item descriptions are as follows: CLIN 0001: Catholic Formation Coordinator Period of Performance: 01 Sep 2009 - 31 Aug 2010 CLIN 1001: Catholic Formation Coordinator Period of Performance: 01 Sep 2010 - 31 Aug 2011 CLIN 2001: Catholic Formation Coordinator Period of Performance: 01 Sep 2011 - 31 Aug 2012 CLIN 3001: Catholic Formation Coordinator Period of Performance: 01 Sep 2012 - 31 Aug 2013 CLIN 4001: Catholic Formation Coordinator Period of Performance: 01 Sep 2013 - 31 Aug 2014 STATEMENT OF WORK 1.0 SCOPE OF WORK: Contractor is responsible for the overall development, programming, and implementation of the religious education, spirituality, and sacramental preparation components of the Langley AFB Catholic Chapel Program. 2.0 HOURS/UNITS OF WORK: 2.1 Contractor services will be provided four (4) days per week (Sunday - Saturday) with flexible hours; minimum of 12 hours per week. The Formation Director (Catholic Chaplain) or his designate will approve hours of availability. 3.0 CONTRACTOR GENERAL RESPONSIBILITIES: 3.1 The Formation Program will conform to the official teachings of the Roman Catholic Church, especially as found in the Catechism of the Catholic Church and publications of the American National Conference of Catholic Bishops, especially the National Directory for Catechesis. In addition, the Program will follow the directives of the Rite of Christian Initiation of Adults. 3.2 The Formation Program will encompass: 3.2.1 Religious education at all levels, pre- K through adult. 3.2.2 Catechumenate and Rite of Christian Initiation, including both youth and adults. 3.2.3 Preparation for the Sacraments: 1st Communion, 1st Reconciliation, and Confirmation. 3.24 Appropriate retreats and occasional speaker series. 4.0 CONTRACTOR SPECIFIC RESPONSIBILITIES: 4.1 Attends periodic formation meetings with Religious Education, LifeTeen/Edge and RCIA program Coordinators, and Catholic Staff meetings as directed by the Formation Director (Catholic Chaplain). 4.2 Supervises and consults with the Program Coordinators/Contractors who are the onsite facilitators for Religious Education, Edge, LifeTeen and Rite of Christian Initiation of Adults programs. Ensures the Coordinators are familiar with all chapel operating instructions including facility usage, and resource utilization and protection. 4.3 Ensures that all formation activities and teaching materials are in accord with the doctrines and practice of the Roman Catholic Church and have the approval of the Catholic Formation Director or his designate. 4.4 Coordinates all activities with directors of appropriate social, music, and liturgy programs. 4.5 Recruits and adequately trains volunteers to staff the Formation Program. 4.6 Ensures that Program Coordinators support the logistical needs of the teaching staff in a timely manner and maintain adequate supplies of curriculum and ministry materials. 4.7 In coordination with the Pastoral Coordinator, the Formation Coordinator is responsible for: proper scheduling (facility requests) and securing of facilities; arranging transportation needs of programs; submitting purchase requests in accordance with local policy. 4.8 Maintains a continuity file for the entire Formation Program and all after-action reports. 4.9 Provides a copy of program publicity to the Pastoral Coordinator in accordance with local policy. 4.10 Oversees the RCIA Coordinator in the preparation of catechumens and the associated retreats in keeping with the Rite of Christian Initiation and guidance of the North American Forum on the Catechumenate. 4.11 Prepares and coordinates sacramental celebrations of 1st Communion, 1st Reconciliation, Confirmation and RCIA. Assures all sacramental records are sent to the Archdiocese for the Military Services. 4.12 Provides for an on-going area Adult Education Program on a weekly, monthly or quarterly basis as determined by the Formation Director or his designate. Maintains a list of available area resource persons and materials. 4.13 Suggests opportunities for spiritual renewal. Provides guidance and logistical support for all approved evangelization programs. 4.14 Provides schedule of hours of availability to the Catholic Formation Director and Pastoral Coordinator. 4.15 Is available for a minimum of 3 hours per day, four days per week (3 weekdays + Sunday) to provide services as required such as: 4.15.1 Answers questions and inquiries 4.15.2 Facilitates registration 4.15.3 Provides resources and supplies 4.16 Develops and tracks the Formation Program. The proposed budget must be developed and submitted to the Catholic Advisory Board (CAB) by 1 April of each year. 4.17 Develops a proposed 12 month calendar of events and scheduled classes to be submitted to Formation Director and Catholic Advisory Board by 1 July. 4.18 Maintains the required AF 1270 database and submits a report to the Langley Chapel 1270 monitor by the 3rd day of the month. 4.19 Inventories resources and equipment and submits a report to the Catholic Advisory Board by 1 June of each year. 4.20 Conducts an on-going registration and maintains a database of teachers, aides, students, RCIA candidates, sponsors and staff. Updated lists are to be submitted quarterly to the Pastoral Coordinator. 4.21 Attends the Catholic Advisory Board's monthly meetings and submits a status report. 4.22 Attends the chapel staff meeting weekly, with an update of activities. 4.23 Visits and evaluates each scheduled religious education class at least once per year. 5.0 EDUCATION, EXPERIENCE AND OTHER SIGNIFICANT REQUIREMENTS: 5.1 Documented proof of the following is preferred: a bachelor degree in theological studies or education and a minimum of two years experience in a Catholic Religious Education Program, and 2 years experience as a coordinator. 5.2 Individual must exhibit a positive, self-directed, and motivated attitude with the breadth and depth of knowledge necessary to accomplish day-to-day coordination, while demonstrating a familiarity with and commitment to the Catholic faith and practice as articulated by the documents of the Second Vatican Council and subsequent statements of the American National Conference of Catholic Bishops. 5.3 Ability to work unsupervised in a fluid and sometimes-stressful environment with staff, parents and children. Possess an ability to maintain healthy, functional interpersonal relationships with co-workers and customers. Maintains professional standards of conduct and public decorum, including but not limited to dress and grooming. 5.4 Satisfactorily completes and understands the expectations of "Protecting God's Children" training, better known as VIRTUS (Latin for Virtues). 5.5 Must be able to communicate the needs and goals of the Formation Program to chapel staff, volunteers and customers. 5.6 Must be able to do word processing in a Windows environment and maintain one or more databases and other files, records and reference lists as required by the Catholic Chaplain. 6.0 SECURITY REQUIREMENTS: 6.1 Physical Security: Contractor shall be responsible for safeguarding all Government property provided for use. At the end of each workday, ensure Government facility, equipment and materials are secure. 6.2 Pass and Identification: Contractor shall obtain and display all Government and contractor identification as required. Contractor shall return all Government issued identification and vehicle passes at contract expiration or termination thereof. The 1st Contracting Squadron will provide base access to all parties identified on an access authorization listing, provided by the vendor, prior to the start of any work. 6.3 Traffic Laws, Weapons, Firearms and Ammunition: Contractor shall comply with all base traffic regulations. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Langley AFB. 6.4 Key Control: Contractor shall safeguard and ensure Government furnished keys are not lost, misplaced or used by unauthorized individuals. Contractor shall not duplicate any Government furnished key and shall report immediately to the Pastoral Coordinator the loss thereof not later than the next workday. 6.5 Background Investigation: The Contractor must have the proper criminal history background checks before receiving the contract to include the National Agency Check Investigation (NACI) and State Criminal History Repositories (SCHR) and an Installation Records Check (IRC) which includes Medical Records check and Security Police check. Forms to be completed are: 6.6.1 Installation Records Check 6.6.2 National Agency Check 6.6.3 State Criminal History Repositories Check Please see the Chapel Background Check Monitor (Chaplain Assistant) for the forms. Forms will be returned to the Background Check Monitor, who will initiate the process. 7.0 GOVERNMENT FURNISHED EQUIPMENT AND SERVICES: 7.1 The Government shall furnish all equipment (desk, office furniture, computer, printer, telephone, etc) necessary for the performance of duties. The Contractor shall be responsible for safeguarding all equipment. 8.0 PRIVACY ACT: Work on this contract may require that Contractor have access to Privacy Information. Contractor Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a, and applicable agency rules and guidance. 9.0 PERFORMANCE EVALUATION: The Quality Assurance Evaluator (QAE) for this contract is the Catholic Priest. The Contractor will consult with the Contracting Officer for resolution of problems or questions about this statement of work. 9.1 Rates of Pay: Rates will be based on a competitive sealed bid. Bids submitted will contain the individual's name and price quote for a monthly unit rate. The Contractor will be responsible for filing any applicable social security, federal, or state taxes. Contractor will be required to submit monthly invoices to the QAE before payment is made. 9.2 Contractor will be evaluated periodically by the Catholic Priest, who will assess his/her work and make necessary adjustments. The Contractor will be given the opportunity to discuss all criticisms or write his rebuttal. 9.3 The Contractor may be required to meet with the Quality Assurance Person(QAP) and/or the Contracting Officer. Meetings shall be as often as necessary as determined by the Contracting Officer. 10.0 TRAINING: 10.1 Specialized training to enhance job skills for this position is authorized if approved by the Wing Chaplain or his designee. Past Performance is deemed relevant and recent if it involves religious event coordination within the past three years.
