Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0350 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33. This acquisition is set aside for small business under NAICS code 334517, Size Standard 500. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: CLIN 0001, 1 EA HUESTIS COMPU-CUTTER III MODEL CC3-1000 COMPUTERIZED BLOCK CUTTING SYSTEM; CLIN 0002 1 EA CBC DICOM; CLIN 0003 1 EA ANTHRO CART CC3-36W. Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics: System should include:Cutter, computer, custom software, printer, digitizer and an optional label printer. Accuracy:Designed as computerized, four-axis cutter. Planning/cutting accuracy must be within + 1mm. Has built-in checks to minimize operator error. Allows review of outline on monitor. Has drawing package feature allowing for easy image manipulation and correction. Performs cut path simulation prior to actual block cut. All functions/set ups completely computer controlled. Radiograph printout verifies tracing. Template printout facilitates mounting shielding block on tray. Treatment Planning Interfaces: Must allow easy transfer of digitized block outlines from treatment planning systems. Eliminates redundant input of block outlines from radiographs. Allows imported outlines to be cut immediately or stored for later use. Radiograph Digitizing: Radiograph is aligned on scribed crosshairs and traced with a mouse. Windows-based (XP or later) program digitizes and stores data and patient files for easy reference or modifications. Easy input of patient/block information. Preset operational function keys. User may select any cut path to connect multiple block outlines. Hot wire cutting: Image can be moved to any position on the block to allow for the conversation of styrofoam use. Cut speed of 0.45-inch/sec (1 cm/sec) or faster. Cutter is safely enclosed in cutting bay. Adjustable temperature for variations in foam densities. Safety interlock for open door position. Custom defaults: S.F.D., S.A.D., S.T.D. Distance changes can be factored in after digitizing. Determines smallest block required for shape or combination of shapes. Quick modificaitons: Ability to flip image for fast AP/PA OR L/R lateral blocks, or change any parameter without redigitizing. Sized for tabletop use: Should have dimensions of approximately 30 inches (76.2 cm) tall x 16 inches (40.6 cm) wide x 30 inches (76.2 cm) deep. Weight should not exceed approximately 60 lbs (27.27 kg). Anthro Cart should house the Compu-cutter III as a single unit with all peripherals. Delivery date is 09/30/2009. Place of delivery and acceptance is Brooke Army Medical Center, Bldg 3600, 3851 Roger Brooke Dr, Ft Sam Houston TX 78234. FOB destination is preferred. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Offers will be evaluated according to 52.212-2. Evaluation factors at 52.212-2 (a) are Lowest price, Technically acceptable (compliance with the requirements of FAR Clause 52.211-6 if vendor offers an Equal Product). Vendors shall include a completed copy of FAR provision 52.212-3 as outlined therein. FAR clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition to include the following addenda: Defense Federal Acquisition Regulation Supplement (DFARS)252.232-7003-Electronic Submission of Payment Requests and DFARS 252.211-7003. Therefore, all quotes submitted in response to this RFQ must remain firm through 30 Sep 09. The closing date and time of this solicitation is 14 Aug 2009 @1:00pm CST.
