Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for FA4800-09-JD-2004. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-12. This acquisition is 100% set-aside for small businesses. The associated NAICS code is 813110 with a $7,000,000.00 size standard. This RFQ contains 5 line items. The line item descriptions are as follows: CLIN 0001: Protestant Religious Education Coordinator Period of Performance: 01 Sep 2009 - 31 Aug 2010 CLIN 1001: Protestant Religious Education Coordinator Period of Performance: 01 Sep 2010 - 31 Aug 2011 CLIN 2001: Protestant Religious Education Coordinator Period of Performance: 01 Sep 2011 - 31 Aug 2012 CLIN 3001: Protestant Religious Education Coordinator Period of Performance: 01 Sep 2012 - 31 Aug 2013 CLIN 4001: Protestant Religious Education Coordinator Period of Performance: 01 Sep 2013 - 31 Aug 2014 STATEMENT OF WORK 1.0 SCOPE OF WORK: The Contractor shall coordinate and provide support for all religious education programs for the Langley AFB Chapel Protestant program. 2.0 HOURS/UNITS OF WORK: A minimum of 25 hours per week will be required, with at least 12 in-office hours. Other than office hours will be coordinated with the Senior Protestant Chaplain or his/her authorized representative. 3.0 CONTRACTOR GENERAL RESPONSIBILITES: 3.1 Works closely with the Religious Education Chaplain in planning, developing and implementing the total Protestant Religious Education Program. Primary attention will be given to the Sunday School program, Awana, Vacation Bible School (VBS), teacher training, and adult education. 3.2 Knows key workers in all services and insures appropriate coverage of Religious Education activities. 3.3 Be liaison for all Religious Education activities. 3.4 Keeps records of various Chapel programs and activities. 4.0 CONTRACTOR SPECIFIC RESPONSIBILITES: 4.1 Recruits, trains, oversees, and coordinates all Protestant Religious Education programs, to include (but, not exclusive of): 4.1.1 Sunday School 4.1.2 Awana 4.1.3 Children's Church 4.1.4 Vacation Bible School (VBS), in conjunction with the Catholic community. 4.2 Provides needed curriculum, supplies, and audio/visual equipment for all Protestant Religious Education programs. 4.3 Submits facility request forms for all Protestant Religious Education programs. 4.4 Develops, annually, a Protestant Religious Education calendar, Ministry Action Plan (MAP) and budget. 4.5 Maintains an up-to-date listing of all Protestant Religious Education volunteers. 4.6 Prepares and coordinates Protestant RE fund requests. 4.7 Coordinates and plans annual Harvest Fest. 4.8 Must maintain model decorum with special attention to dress and conduct themselves in a manner appropriate for religious services and programs. 4.9 Dresses appropriately for a professional atmosphere. 4.10 Attends monthly Protestant Advisory Council meeting and reports the status of all religious education programs. 4.11 Attends weekly chapel staff meetings. 5.0 SECURITY REQUIREMENTS: 5.1 Physical Security: Contractor shall be responsible for safeguarding all Government property provided for use. At the end of each workday, ensure Government facility, equipment, and materials are secure. 5.2 Pass and Identification: Contractor shall obtain and display all Government and contractor identification as required. Contractor shall return all Government issued identification and vehicle passes at contract expiration or termination thereof. The 1st Contracting Squadron will provide base access to all parties identified on an access authorization listing, provided by the vendor, prior to the start of any work. 5.3 Traffic Laws, Weapons, Firearms and Ammunition: Contractor shall comply with all base traffic regulations. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their privately owned vehicle while on Langley AFB proper. 5.4 Computer: Contractor will be issued a Common Access Card to access government furnished computer. 5.5 Key Control: Contractor shall safeguard and ensure Government furnished keys are not lost, misplaced or used by unauthorized individuals. Contractor shall not duplicate any Government furnished key and shall report immediately to the Senior Protestant Chaplain the loss thereof not later than the next workday. 5.6 Background Investigation: The Contractor must have the proper criminal history background checks before receiving the contract to include the National Agency Check Investigation (NACI) and State Criminal History Repositories (SCHR) and an Installation Records Check (IRC) which includes Medical Records check and Security Police check. Forms to be completed are: 5.6.1 Form 2583 - Installation Records Check 5.6.2 Form 389-2 - National Agency Check 5.6.3 State Criminal History Repositories Check Please see the Security Manager for the forms. Forms will be returned to the Security Manager, who will initiate the process. 6.0 EDUCATION, EXPERIENCE, AND OTHER REQUIREMENTS: 6.1 A minimum of four year of college desired. 6.2 Five years of teaching religious education classes. 6.3 Three years experience as a Religious Education Coordinator. 6.4 Regular participation in a Protestant Church or Chapel. 6.5 Recommendation from the pastor of that church or chapel. 6.6 Experience working with other denominations. 6.7 A working knowledge of a variety of religious education materials is required. 6.8 Must be able to motivate and recruit volunteers. 6.9 Must interview with the Senior Protestant Chaplain or his/her designee to determine if applicant meets above requirements. 7.0 GOVERNMENT FURNISHED EQUIPMENT AND SERVICES: 7.1 The Government shall furnish all equipment (desk, office furniture, computer, printer, telephone, etc) necessary for the performance of duties. The Contractor shall be responsible for safeguarding all equipment. 8.0 PRIVACY ACT: 8.1 Work on this contract may require that Contractor have access to Privacy Information. Contractor Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a, and applicable agency rules and guidance.. 9.0 The contract manager may be required to meet with the Quality Assurance Personnel (QAP) and the contracting officer. Meetings shall be as often as necessary as determined by the contracting officer. 9.1 Rates of Pay will be based on a competitive sealed bid. Bids submitted will contain the individual's name and price quote for a monthly/yearly rate. The Contractor will be responsible for filing any applicable social security, federal, or state taxes. Contractor will be required to submit monthly invoices to the QAE before payment is made. 9.2 The Contractor may be required to meet with the Quality Assurance Person (QAP) and/or the Contracting Officer. Meetings shall be as often as necessary as determined by the Contracting Officer. 10.0 OTHER ABSENCES: 10.1 The Contractor shall notify the Contracting Officer or Quality Assurance Evaluator when absences will occur. 10.2 Contractor will either personally perform the above described services or will cause them to be performed by a substitute(s) of equal or higher proficiency, which are approved by the Contracting Officer or Senior Protestant Chaplain in advance. 11.0 TRAINING: 11.0 Specialized training to enhance job skills for this position is authorized if approved by the Wing Chaplain or his designee. Technical and Past Performance will be pass/fail based on compliance with the Statement of Work. Past Performance is deemed relevant and recent if it involves religious education coordination within the past three years.
