- Checklist Attachment VENDOR FRIENDLY REMINDER for HSTS01-09-R-00134 AMENDMENT 04 (AMD04) 1) This amendment is issued to: (a) remind potential offerors of the closing date for submission of proposals on solicitation number HSTS07-09-R-00134, (b) advise that the closing TIME is now 2 P.M. Eastern Standard Time and to (c) Remind all potential offerors that if they believe that they still have unanswered questions, please review to ALL previous Amendments. The RFP closes on Monday, August 10, 2009 – and it is the responsibility of the vendors to assure that all required components of the RFP are received in a timely fashion. THE TIME FOR RECEIPT OF PROPOSALS HAS CHANGED FROM 10 A.M.TO NO LATER THAN 2 P.M. Eastern Standard Time (EST) to the Contracting Facility at: Contracts and Procurement Division Federal Air Marshal Service Reference: HSTS07-09-R-00134 200 West Parkway Dr Ste 300 Egg Harbor Twp, NJ 08234 Attention: Doretta Chiarlone Phone: 609-813-3363 E-mail: Doretta.Chiarlone@dhs.gov 2) No hand carried proposals or clothing samples will be accepted. 3) Reminder that the Amendment 02 revised Excel spreadsheets, specifically Tab 2 is the only pricing spreadsheet that will be accepted. Use this revision to Tab 2 ‘Pricing for Proposal Evaluations’ (posted 7/24/09). No other/earlier versions of this Excel Spreadsheet will be accepted because the earlier versions do not reflect the current estimated quantities and therefore would be incorrect. 4) Reminder that all offerors should acknowledge receipt of the Solicitation Form SF 1449 (7/10/09) and all Amendments (AMD 01, 7/22/09), (AMD 02, 7/24/09), (AMD 03, 7/29/09) and (AMD 04 8/5/09). For the solicitation, the SF 1449 (pg. 4 of 100) should be signed in block 30a. A letter (word document) signed by authorized company official is acceptable stating that each Amendment (AMD01 through AMD 04) was received.
