- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81NTE91908501 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS code 339114, Size STD 500. Sole Source The requirements in this solicitation: 1. Keyboard Arm Assemby (P/N #: 56.0270.00) - Qty 22 2. Installation - Qty 22 MFG: A-DEC Inc. For Vendors shall comply with FAR clauses throughout this solicitation can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, NLT 30 September 2009, shipping shall be FOB destination to Brooke Army Medical Center, Fort Sam Houston, TX 78234. Acceptance Location: Brooke Army Medical Center, USA Dentac HQ, Blg 1029, 2410 Stanley Road, Fort Sam Houston. TX 78154. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Evaluation of Quotes: Offers will be evaluated according to Far 52.212-2.
