- RFQ Response Form COMBINED SYNOPSIS - SOLICITATION FOR COMMERCIAL ITEMS (IAW FAR 12.603 (c)) Solicitation POCH9012 posted July 8th, 2009 has been cancelled. Please submit responses referencing solicitation POCH9012A (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and FAR 13.5. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is POCH9012A and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-34. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 335121 with an employee size standard of 500. (v) Contractors shall submit a lump sum quote for the following: This is a brand name or equal acquisition of Grandrich 26" Full Spectrum Flex Desk Lamp in Black, Energy Star Compliant, Model Number ES-201 BLK 0001-Contractor will provide Desk Lamp, black in color, 700 each, on the attached request for quote sheet. Desk lamps must have a flexible gooseneck to allow the user to shine the light where needed, durable plastic construction, lamps must include 27 watts full spectrum bulbs at a minimum, full spectrum included light bulb, maximum height not to exceed 30" and must be energy star compliant. This acquisition is a Brand Name or Equal in accordance with FAR 52.211-6, manufacturer part numbers are offered as examples only. Vendors must quote the brand name or the equivalent of the brand name to be considered for award. To be considered for award offerors of equal products must include meet all of the salient characteristics listed above. Vendors must clearly identify the offered item by brand name, make or model number, provide descriptive literature such as illustrations; specification sheets that clearly demonstrate that the offered item meets the salient characteristics listed above. Vendors must clearly describe and all modifications to the offered item that will make it conform to the salient characteristics listed above. The contracting Officer will evaluate equal products on the basis of the information provided by offerors. Unless the offeror clearly indicates in its quote that the item being offered is an equal product the offeror shall provide the brand name item specified in this solicitation. All responsible sources may submit a quotation/proposal, which shall be considered. (vi) Delivery of items is FOB destination. Items to be delivered in accordance with the schedule below to the loading dock at the McCormack Post Office and Courthouse. Garage can accommodate a box truck measuring 10'6". •October 15th, 2009200 Lamps •November 1st, 2009500 Lamps (vii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1, Instructions to Offerors-Commercial. The Government will award a contract resulting form this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Technical capability of the item to meet the government's requirement; 2.
