- ONLY WRITTEN REQUESTS FOR COPIES OF THE SOLICITATION WILL BE HONORED. IMPORTANT: TELEPHONE REQUESTS WILL NOT BE HONORED. The contractor shall provide all supervision, labor, tools, equipment, transportation and materials required to perform the work listed as specified in the Statement of Work. Work includes, but is not limited to: Dividing an existing two-person, 190 SF (Tri-Care) office located in the Base’s Healthcare Clinic into two single-person offices: a 110 SF Tri-care office and an 80 SF Physical Therapy office. This work is to be accomplished by constructing an acoustic/metal stud wall between the two offices; furnishing a new, large, sliding window in the common wall between the new Physical Therapy office and the adjoining Physical Therapy exercise/treatment area: and replacing and relocating an existing sink in the Tri-care office to the new Physical Therapy office side. Contractor will be selected based on the lowest price technically accepted. All responsible sources that submit a written quotation will be considered. GOVERNMENT FURNISHED MATERIALS: NONE Performance Period is 30 - 40 days. Work to be performed Dec 07, 2009 – Jan 15, 2010. SUBMITTALS DUE UPON AWARD may include but are not limited to: The Contractor shall provide submittals for review & approval for the following items: 1) Steel Door Assy per Appendix A Task #1, 2) Metal Stud Wall per Appendix Task #6, 3) Provide cut-sheet, shop drawing and installation instructions for window per Appendix A Task #7, 4) Provide cut-sheet for Venetian blinds per Task #7, 5) Provide name of firm that will build cabinet, to include shop drawings and samples of vanity cabinetry and sink assy, hardware that will be required for power to flow from respective transformers through conduit in the new wall and exit the wall to the dispensers per Appendix A Task #8, 6) Carpet Tiles Installer per Appendix A Task #9a, 7)Prescriptive Requirement of Furnished Conditioned Air to Offices to include name of Mechanical sub-Contracting firm per Appendix A Task #9h. Manufacturer catalogs, standard schedules, performance charts, instructions, brochures, diagrams, etc., in addition to references and qualifications. Lack of adequacy of references shall be at the sole discretion of the Government and may constitute grounds for disqualification. CONTRACTOR SHALL COMPLY WITH requirements of: applicable Coast Guard Safety Regulations, federal, state, and local regulations pertaining to water, air, and noise pollution. ADDITIONAL SPECIFICATIONS OR REQUIREMENTS ARE CONTAINED IN STATEMENT OF WORK THAT WILL BE INCLUDED WITH REQUEST FOR QUOTE DOCUMENTS WHEN THEY ARE ISSUED ON APPROXIMATELY AUGUST 04, 2009. A SITE VISIT WILL BE HELD AUGUST 25, 2009, APPROXIMATELY TWO WEEKS BEFORE QUOTE IS DUE. (Site visits are not mandatory, but are highly encouraged.)
