- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0312 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS code 334516, Size STD 500. The requirements in this solicitation: 1. CYTOCENTRIFUGE (GRAM SLIDE STRAINER MODEL 7320) Qty 1 2. CYTOCENTRIFUGE (HEMOTOLOGY SLD STRAINER MODEL 7150) QTY 1 MFG: WESCOR INC Vendors offering other than the brand name product shall meet or exceed the following Salient Charteristics: " Model 7320 Accessories Included: 12 slide carousel (2 each), drain tube (1 length), 5.0 liter bottle assembly for reagent D (1 each), nozzle maitenance kit (1 each), decolorized with safranin reagent (1 bottle), acetoner decolorizer with safranin reagent (1 bottle), iodine reagent (2 bottles), crystal violet reagent (2 bottles), deionized water (2 bottles), methanol reagent (2 bottles), preventive maintenance chart (2 year supply), users manual and material safety data sheets. " Model 7150 Accessories: 12 slide carousel (2 each), drain tube (1 length), 5.0 liter bottle assembly for reagent D (1 each), nozzle maitenance kit, reagent A buffer PH 6..8(dilute to 5L) (1 bottle), reagent A PH 7.2 30ml (dilute to 5L) (1 bottle), reagent B Thiazin Stain, 500ml (2 bottles), reagent C Eosin Stain, 500ml (2 bottles), methanol reagent, 500ml (2 bottles), preventive maintenance chart, pad of 24 sheets, afrofix additive for methanol reagent, 135ml (1 bottle), 5.0 liter bottle assembly for buffer (reagent A), and users manual. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Bayne Jones Army Community Hospital, FORT POLK, LA 71459. Acceptance Location: Bayne Jones Army Community Hospital, US Army Meddac PBO, 7606 Virginia Ave, Bldg 4372, Fort Polk, LA 71459.
