Description
Performance work statement for lowering Giant Voice system poles This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation FA4877-09-Q-0340 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33, Defense Circular Number 20090715-1 and Air Force Acquisition Circular 2009-0318. (iv) This procurement is being issued as a HubZone Set-Aside. The NAICS code for this requirement is 237130. (v) The contractor shall provide all personnel, labor, transportation, supervision, materials, equipment, and management: to provide the following: Perform pole cutting services from existing standing poles from various locations on Davis-Monthan AFB, AZ. The contractor shall lift existing wooden poles out of the ground, remove 10 feet from the bottom of each 60 foot pole, and replant the poles in the same hole. There are four wood poles, each at a different location on DMAFB, that require lowering. Each wood pole has a speaker and control cabinet attached to it, and the contractor shall not remove or reposition the speakers or control cabinets. The contractor shall remove all waste products from Davis-Monthan AFB, resulting from the contractors work, including the 10 feet of pole removed from each pole. All work shall be done IAW attached Performance Work Statement (PWS). Clarifications to be discussed at site visit. The provision at 52.212-1--Instructions to Offerors -- Commercial, applies to this acquisition. (ix) 52.212-2--Evaluation -- Commercial Items applies. Quotes will be evaluated on the lowest price technically acceptable, and the quote that will yield the best value to the Government. (x) Offerors must include a completed copy of the provision at, 52.204-8-- Annual Representations and Certifications and submit with its offer. (xi) Clause 52.204-9--Personal Identity Verification of Contractor Personnel - contractor shall comply with agency personal identity verification procedures identified in the contract that implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201. Site visit is Mandatory for interested contractors submitting proposals. Proposals shall be due to the 355th Contracting Squadron, 3180 S. Craycroft Rd.
