- Company Info Sheet. Sole Source Letter Travis AFB intends to award a sole source purchase order for web based residency management to Myevaluations.com, Inc. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-34, effective 01 Jul 2009, DCN 20090115, and AFAC 2009-0318 effective Mar 18, 2009. This acquisition will be unrestricted; the North American Industry Classification System (NAICS) code is 611430. The business size standard is $7.0M. The Federal Supply Class (FSC) is U012. The Standard Industrial Classification (SIC) is 9411. This request for quotation consists of the following: CLIN 0001: Web Based Residency Management; Conists of: Class 2 - Monthly Family Medicine: Service Fee for 31-50 Residents/Fellows; Class 1 - Monthly Radiology: Service Fee for 30 pr less Residents/Fellows; Class 1 - Monthly Transitional: Service Fee for 30 or less Residents/Fellows; Class 1 - Monthly Oral & Maxillofacial Surgery: Service Fee for 30 or less Residents/Fellows; ACGME 2008-2009 Site Survery v.3 Setup; Medical Training Management (evaluations, procedures, patient logs, duty-hours, conferences, and demographics); My Portfolio document management; AMION/Scheduling. Period of Performance: Basic Year: 01 OCT 09 - 30 SEP 10 Option Year 1: 01 OCT 10 - 30 SEP 11 Option Year 2: 01 OCT 11 - 30 SEP 12 Option Year 3: 01 OCT 12 - 30 SEP 13 Option Year 4: 01 OCT 13 - 30 SEP 14 Award shall be made in the aggregate, all or none. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
