Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions. It is recognized that any cost information provided by the contractor is a preliminary estimate for planning purposes and may be marked as proprietary; the contractor will not be bound to these estimates. Department of Veterans Affairs, VISN 3 (Hudson Valley, Northport, and Bronx campus) are seeking potential sources to provide advisory and assistance services to improve veteran patient centered care and satisfaction within the Medical Centers. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of business, e.g. small, Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, Small Business HUBZone business, other than small business, or Large Business and (i) a capability statement (j) Resumes of the personnel who you propose will provide the advisory and Assistance services; (k) Information including a list of similar contracts that have been performed within the past five years including name and phone number of contact person for each contract. Please note that these contacts will be called and used as a reference. The capability statement should also include how well your company can meet the following criteria: 1. Proven success in the following areas: "Development of patient-centered care principles with regards to space planning and architectural improvement; "Collect and analyze progress assessment information and tailor facility specific plans for improvement; "Cultural transformation of an organization to one that focused on patient-centered care principles; "Patient satisfaction improvement strategies. 2. Contract requirements will include providing training manuals, sample policies and procedures, regional meetings and/or conferences with other facilities with contractor implemented programs, conference strategy calls, leadership readiness sessions, organizational assessments, assessment recommendations to each facility, and detailed project management plans for each facility to implement a veteran patient focused care model. Provide information sessions for all employees, managers, board members and medical staff on patient-centered approaches. 3. Team members would consist of experienced and knowledgeable experts in the field of JCAHO, Quality Assurance, Consulting, Organizational Development, Healthcare, Business and Industry. 4. Contractor will be able to provide various assessment sessions in leadership, project management and planning to include: "Strategies for improving veteran patient-centered care and satisfaction. Identifying and developing common goals, and contractor provided practices for improvement based upon successful previous experiences with patient focused care implementations. "Contractor personnel on site at each facility (Northport VA, Castle Point VA, Montrose VA, and JJP VA-Bronx) for facility tours, reviews of performance measures and human resource practices, focus groups with patients and staff, provision of initial feedback and recommendations for improvement and implementation of a patient centered care model. "Provide a project Management Plan detailing the project team, schedule of activities, and identify the contractor staff to provide further assessment and advisory and assistance services. Please provide any information that will demonstrate your company's ability and expertise to perform the services described herein. Important Information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The VA will not return pictures, documents, and graphs or inter-related requested information. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the Central Contractor Registration (CCR located on the web at www.ccr.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.
