- Request for Quote Sheet COMBINED SYNOPSIS - SOLICITATION FOR COMMERCIAL ITEMS (IAW FAR 12.603 (c)) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and FAR 13.5. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M3E79134A001 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32 effective 14 May 09. (iv) This acquisition is unrestricted for full and open competetion. The associated NAICS code is 335222 with an employee size standard of 1000. (v) Contractors shall submit a lump sum quote on the attached request for quote sheet for the following: 0001-Contractor shall provide Microfridge Model #MF5 9TP, brand name or equal product, 220 each, black, two door refrigerator/freezer with touchpad microwave oven with the following features at a minimum: Fridge Capacity: 3.46 cu. ft. Freezer Capacity: 1.38 cu. ft. Total Capacity: 4.8 cu. ft. Internal Safe Plug TM Technology: Yes Doors: 2 Defrost Type: Frost Free Freezer Type: Freezer Microwave Watts Output: 850 Height: 58.63 in Width: 18.62 in Depth: 22.62 in Amps: 12 The microwave must be attached to the unit 0002-Contractor shall remove 213 old appliance combos from buildings 464, 466, and 473 (71 units per building) and relocate old appliances to an on-base storage house located at 1439 Cobbs Ave, Charleston AFB, SC. 0003-Contractor shall deliver and install 213 new appliances in buildings 464, 466, and 473 (71 units per building) plugging the new appliances in to ensure they are operational. The contractor is responsible for all moving and lifting of the appliances. The contractor is responsible for any damage to the appliances during moving and installation. The contractor shall deliver the remaining 7 new appliances to building 629 on Charleston AFB, SC. Installation of these 7 new units shall not be required. All responsible sources may submit a quotation/proposal, which shall be considered. (vi) Delivery of items is FOB destination. Items must delivered and installed 30-45 calendar days of award. (vii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1, Instructions to Offerors-Commercial. (viii) FAR 52.212-2 Evaluation-Commercial Items Evaluation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: 1.Technical acceptability of the item offered to meet the Government requirement. 2. Price- The Government will evaluate offers for award purposes or the total price for the requirement. (ix) All offerors shall include a completed copy of FAR 52.212-3, Offeror Representation and Certifications--Commercial Item or complete electronic annual representations and certifications at http//orca.bpn.gov. (x) The clause at 52.212-4 Contract Terms and Condition-Commercial Items, applies to this acquisition.
