- Sole Source Letter for American Lung Association requirement. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4539155A004; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-34, effective date July 01, 2009; Defense DCN 20090115, effective date 15 Jan 2009; and AFAC 2009-0318 effective date 18 Mar 2009. The North American Industry Classification System (NAICS) code is 624190. The business size standard is $7,000,000.00. The Federal Supply Class (FSC) is U099. The Standard Industrial Classification (SIC) is 8299. Travis AFB intends to award a Sole Source requirement for telephone support and counseling for smoking cessation program, to the American Lung Association, with 4 option years. CLIN0001- Telephone Support and counceling for smoking cessation program. 1 year. CLIN0002- Option year 1. CLIN0003- Option year 2. CLIN0004- Option year 3. CLIN0005- Option year 4. ALA Shall provide a live operator and a toll-free tobacco cessation quit-line. Each caller recieves individualized tobacco cessation counseling and a free packet of information outlining the cessation process. Services provided Monday-Friday 0900-2300. Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items (June 2008). FAR 52.212-2 Evaluation -- Commercial Items (Jan 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
