- This is a notice of the Governments intent to solicit, negotiate and award a sole source contract to, Creative Health Care Management Inc. 1701 American Blvd. E STE 1, Minneapolis, MN 55425 for the Relationship Based Care Assessment and Educational Training. Contractor shall furnish two Board Certified Registered Nurses for assessment, training and implementation of the Relationship-Based Care Delivery system to include 28 training manuals and certificatin of Twenty One Continuing Education Units (CEUs. Assessment and training shall be covered during the period 10 - 14 August 2009 to include Assessment, Kickoff Wave I, The Caring Model Facilitator Program, and Relationship Based Care Orientation for Leadership This is not a set-aside for small business. This notice is an urgent requirement for Womack Army Medical Center, 2817 Reilly Rd., Fort Bragg, NC 28310. Parties interested in future announcements shall provide detailed information of their capabilities and certifications to clearly meet the requirements stated above.
