- NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention, National Institute for Occupational Safety and Health (NIOSH) Pittsburgh PA, hereby announces its intent to issue a purchase order on a sole source basis to the Alaska Trauma Registry (ATR), Alaska Department of Health & Social Services to conduct through surveillance for all injuries resulting in a hospitalization in civilian Alaskan hospitals, through data collection and operating agreements with each hospital. The ATR is the only entity in the State currently providing such service (comprehensive surveillance for severe non-fatal injuries), no other activity provides either the service or possesses the capacity to perform, resources and information from all EMS and hospital sources within the State of Alaska. Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the contracting officer within 15 days from the posting date of this notice, their capabilities statement and pricing information in the format they choose, not to exceed 10 pages. Please forward capabilities and supporting information to James Ambrozic, This project is supported by justification for other than full and open competition to LMS North America, under authority of 41 U.S.C.253(c)(1). Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written item description/item characteristics and pricing information in the format they choose, not to exceed 10 pages. Please forward the item description and pricing information to James Ambrozic, Purchasing Agent, Reference: 00HCCA1G-2009-73787, Center for Disease Control and Prevention, Acquisition and Assistance Field Branch (Pittsburgh), PO Box 18070, Cochrans Mill Road, Pittsburgh. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all item descriptions and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with the Alaska Trauma Registry, 350 Main Street, Room 427, Juneau, AK 99811-0616, as the only source and a purchase order will be issued without any additional notices being posted.
