- The Agency for Healthcare Research and Quality (AHRQ), through the Program Support Center, Division of Acquisitions Managements, intends to award a 12 month sole source contract using simplified acquisition procedures to the CHD Research Associates, 2300 Clarendon Blvd., Suite 1400, Arlington, VA 22201. The purpose of this contract is to conduct further analyses of trends in national and state measures of hospitalization among home health and nursing home patients, and factors affecting these measures. The contractor shall perform the following tasks and activities, not to exceed 500 hours: Using existing (and updating files through latest available calendar year) home health and nursing home patient level files containing either Minimum Data Set (MDS) or Outcomes and Assessment Information Set (OASIS) assessment data merged with Medicare claims data, calculate: All-cause and potentially preventable hospitalization rates, using the AHRQ PQIs methodology and any additional ICD codes specified by the Project Officer, occurring within specified time periods after start of care for 3 groups: (1) Medicare Home Health patients; (2) Medicare Skilled Nursing Facility (SNF) patients and (3) Long stay (i.e., excluding SNF) --All nursing home residents with MDS assessment regardless of being in Medicare SNF episode Estimate actual PQI hospitalization rates and associated costs for home health and nursing home users and for additional subgroups of these users – for example, dual eligibles, patients with ESRD or near end of life, patients with different levels of functional and cognitive impairment, and users living in different geographic regions or being served by different types of agencies/facilities, and for racial/ethnic groups used in the NHQR/NHDR (sample size permitting). The contractor may apply previous work (to be supplied by the Project Officer) on risk adjustment for home health/nursing home-related hospitalization rates, in order to improve the validity of the comparisons. Within the maximum total labor hours for this purchase order, develop initial predictive models for potentially preventable hospitalizations (e.g., logistic and/or other multivariate regression), as appropriate, controlling for patient characteristics, regional market factors (e.g., including Dartmouth index for available years, HCC) and other variables to produce risk adjusted national estimates of hospitalizations and preventable hospitalizations for home health and nursing home users. The proposed simplified acquisition is for services for which the Government intends to solicit and negotiate with only one source. Interested persons may identify their interest by responding to the requirement within 10 days after the date of publication of this notice. A determination by the government not to compete this proposed simplified acquisition based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
