Middleton VA Hospital 2500 Overlook Terrace Madison, Wisconsin 53705 MEMO FOR RECORD SUBJECT: Rehab Therapy Expansion, Pre-Bid/Site Visit Meeting Minutes 1. At 10:00 on 15 July, a site visit was conducted at the Tomah VAMC, Tomah WI for the Rehab Therapy Expansion solicitation. The meeting began at the Tomah VA conference room. John Meyers opened by stating that discussions during this site visit will not change or interpret the Specifications or Solicitation. He further stated that any questions technical in nature would need to be submitted in writing; changes or interpretations of the Specifications and solicitation will only be official when accomplished by an amendment. Provide questions in writing NLT 7/20/2009. 3. The meeting was turned over to Carol Harmon and Kurt Marshaus to provide some generic information. The information provided will be covered again during the pre-construction meeting and is of a nature that will not affect the substance of ones bid. 4. The floor was opened to questions regarding the project specifications and drawings: 5. The question was asked if the clinics would be vacated during construction; Kurt Marshaus stated that they intended to have the ground floor clinics vacated. 6. The question was asked if there would be work hour restrictions; Kurt Marshaus stated that there would be restrictions due to patients on the upper floors for after hours requested work. Normal work hours are 7:00-4:30. 7. The question was asked concerning haul routes for the project; Kurt Marshaus stated that there was only one route since the only access is through the Head House entrance. 8. The question was asked concerning whether the miniature golf would be moved; Kurt Marshaus stated that it would remain; however, it could be relocated for the construction phase and restored at completion. 9. The question was asked if there was any hazardous material in this project; Kurt Marshaus stated that there should be no asbestos or lead paint or any other material present. 10. John Meyers closed the visit by re-iterating that discussions held at the site visit do not modify or interpret the Specifications/Drawings or Solicitation.
