Description
This is a revised market survey. In accordance with Federal AcquisitionRegulation (FAR) 19.202-2, the Space and Naval Warfare Systems CenterAtlantic in Charleston, SC is conducting a market survey (under solicitationnumber N65236-09-R-0095) to identify ALL SMALL BUSINESS (SB) concerns (i.e.small business, veteran-owned small business, service-disabled veteran-ownedsmall business, HUBZone small business, small disadvantaged business, andwomen-owned small business) capable of providing Health System InformationTechnology and Information Assurance (IT/IA) Data Strategy and Governancesolutions several DoD and Federal agencies. This work includes providinginnovative solutions based on "best business practices" and comprehensiveanalysis and assessments of medial data. It includes the ability to exchangepatient health information among clinicians and other authorized entities inreal time and under stringent security, privacy and other protections;authentication and identification of both clinicians and beneficiaries; andnational accessibility of patient's healthcare providers to both healthcareproviders and the patient. The proposed action will be a Firm-Fixed-Price, Indefinite-Delivery-Indefinite-Quantity type contract. The FSC Code is D307. The NAICS Code is 541512 witha size standard of $25M. The period of performance is currently forecasted tobe 21 JAN 2010 through 20 JAN 2011 for the base year. There will be fouroption years and if exercised, will take this procurement action to 20 JAN2015.The Government estimate for this action is $25M for the base year and fouroption years. Interested parties need to identify their interest and capability in responsetothis requirement by 31 JUL 2009 to be considered by the Government. Theresult of this market survey will determine if the acquisition will beset-aside forsmall businesses and competed within one of the small businesssocioeconomic preference programs. To be considered, all potential sources shall be registered in the CentralContractor Registration (CCR). All SB concerns are invited to submit theappropriate documentation, literature, brochures, and references necessary tosupport that they possess the required capabilities necessary to meet orexceed the stated requirements. There is a page limit of ten (10) pages totalon data submitted and must include the following in the order as prescribed: (1) Name and address of firm(2) Size of business, including: average annual revenue for past three yearsand number of employees(3) Ownership, meeting eligibility requirements for small businesssocioeconomic preference programs, such as small business, veteran-ownedsmall business, service-disabled veteran-owned small business, HUBZonesmall business, small disadvantaged business, and women-owned smallbusiness.(4) Number or years in business(5) Two points of contact, including: name, title, phone, fax, and e-mailaddress(6) CAGE and/or DUNS Number (if available)(7) Affiliate information, possible subcontractors (if potential prime)(8) The percentage of work your company can perform with respect to thestated requirements (Do not include any documentation or percentages for anycompany other than your own. This is an analysis of your company?s abilitiesof what it can or cannot perform)(9) A list of customers for relevant work performed in the past five (5)years,including a summary of work performed, contract numbers, contract type, dollarvalue for each contract referenced, how this work demonstrates capability toperform percentages stated in number eight, and a customer point of contactwith phone number (this information is required to verify offeror?sperformance; the government may contact any reference for further validation.)
