- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested for request for information/quote. Solicitation number F1U4M29194A010 is hereby issued as a Request for Quote (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 562211, with dollars not to exceed $12,500,000.00. DESCRIPTION OF REQUIREMENT: The contractor shall provide all management, tools, supplies, equipment, and labor necessary to furnish the following to Eielson AFB, AK: Medical Waste Disposal in accordance with attached Statement of Work. Period of Performance: 1 Oct 09 - 30 Sep 10 (End description). Quotes must be submitted and received no later than 27 July 09 at 4:30pm Alaska Daylight Time. Quotes may be e-mailed or faxed to 907-377-2389 to the attention of primary P.O.C.s listed below. The Government's obligation is contingent upon the availability of funds from which payment for contract purposes can be made. No legal liability on part of the Government for any obligation to contract or payment may arise until funds are made available and until the Contractor receives notice by way of written agreement from the Contracting officer. A firm-fixed price contract is anticipated. Telephone bids will not be processed. Award shall be made only to contractors who have registered with Central Contractor (CCR).
