appendix e appendix c appendix b Questions & Answers THIS MODIFICATION IS ISSUED TO POST Q&As and ATTACHMENTS. THIS MODIFICATION IS ISSUED TO CHANGE THE RESPONSE DATE FROM JULY 16, 2009 TO JULY 29, 2009 @ 11:00AM. This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a fixed priced purchase order at a fixed labor for Virtual Training and Communications for CMS Regional Office Employees. This purchase order will be used to provide a one-time training opportunity for all CMS employees in the ten regional offices. The contract will run for approximately four months. The first 45 days of the contract will consist of the planning and creation of the training module. The next 45 days will consist of the implementation (training to be held in each of the ten regional offices). The final 30 days will consist of the creation of a follow-up report, which will be presented to the project officer. It is anticipated that the period of performance will begin August 1, 2009. The Statement of Work and Deliverables are attached. The North American Industry Classification System Code (NAICS) is 611430. Both the technical and business quotation shall be submitted in hard copy (2) two originals to: Centers for Medicare & Medicaid Services, OAGM/Division of Support Contracts, ATTN: Lucille Lee, Contract Specialist, 7500 Security Blvd., M/S C2-21-15, Baltimore, MD 21244-1850, no later than 11:00 am on July 16, 2009. In addition, an electronic format shall be submitted to Lucille.Lee@cms.hhs.gov and project officer Catherine.Kortzeborn@cms.hhs.gov. For the purpose of establishing timely receipt of a quotation, the time will be established based upon the hard copy submission. Offerors are advised that CMS is a secured facility and therefore, offerors should allow time to obtain the appropriate security clearances (e.g., parking pass, visitor badge, etc...). Vendors are also advised that the CMS mailroom accepts only U.S. Postal Service mail. When utilizing an overnight carrier, you are cautioned that you must allow time for the proposal to be delivered to the exact mail stop noted above by the carrier by the time specified in this RFQ. Responses shall include the following: 1) Technical Understand and Approach; 2) Personnel Qualifications; 3) Past performance information - relevant experience to perform the required services; and 4) fully loaded labor rate and anticipated hours. All responses will be evaluated on a best value basis using the following Evaluation Criteria: 1) Technical Understand and Approach, 2) Personnel Qualifications, 3) Management and 4) Past Performances. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price; the best value (considering price and other factors). All questions concerning this solicitation shall be submitted in writing no later than 10:00am (local Baltimore, MD time) on July 8, 2009. All relevant questions will be addressed in a timely manner, and the Government's response will be provided to all interested parties with copies of both the questions and answers being supplied without reference as to the originating source. Questions submitted by telephone will not be entertained.
