- Questionnaire Wage Determination No. 2005-2419, Rev. 7, dated 05/26/09 Performance Work Statement dated 25 Jun 09 Solicitation The attached solicitation (SF1449 with Attachments) is for Clinical Radiation Oncology Services for the Medical Treatment Facility located at Wright-Patterson AFB, OH. This is a 100% Small Business Set-Aside. The period-of-performance is 1 Oct 2009 through 30 Sep 2010 with 4, 1-year options. Offerors are advised that the cost of any response to, or other cost incurred as a result of this solicitation is at the offeror's own risk. Offerors are cautioned to read the entire solicitation package before submitting a proposal to ensure a complete package is submitted. Information to Offerors/Proposal Instructions can be found in clause 52.212-1 of the solicitation. Information concerning evalation of the proposals can be found in clause 52.212-2 of the solicitation. The North American Classification System (NAICS) code is 621512.
