Description
The Fleet and Industrial Supply Center, Norfolk Contracting Department, Norfolk Office, Norfolk VA, 23511-3392 intends to award Firm Fixed Price Purchase Order on a sole source basis to Staar Surgical Company utilizing FAR Part 13.5 "Test Program for Certain Commercial Items" for (50) Visian ICL (Implantable Collamer Lens) for a base year and four (4) one-year option periods. The statutory authority permitting other than full and open competition is10 U.S.C. 2304(c)(1), only one responsible source and no other supplies or services will satisfy agency requirements as implemented by Federal Acquisition Regulation (FAR) 6.302-1 (b). Delivery is FOB Destination to Naval Hospital Camp Lejeune, Camp Lejeune, NC 28547. The RFQ will be available on or about 16 July 2009 by downloading the documents at http://www.neco.navy.mil or http://www.fedbizopps.gov. The applicable North American Industry Classification System Code is 339113. The size standard is no more than 500 employees. The RFQ solicitation and any subsequent amendments will ONLY be available for download at the above sites; this office will no longer issue hard copy solicitations. By submitting a quote, the vendor is self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred or otherwise ineligible to receive contracts from any federal agency. Failure to do so may represent grounds for refusing to accept the offer. By submission of an offer, the contractor acknowledges the requirement that a prospective awardee must be registered in the CCR database prior to award, during performance, and through final payment of any contract resulting from this solicitation, except for awards to foreign vendors for work to be performed outside the United States. Lack of registration in the CCR database will make a contractor ineligible for award. Contractors may obtain information on registration and annual confirmation requirements by calling 1-888-227-2423, or via the Internet at http://www.ccr.gov.
