Description
The proposed contract action is for commercial item, which the Naval Medical Center San Diego intends to solicit this requirement on a sole source basis to Audiometrics under the authority of Far 13.106-1(b). This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-09-T-0312 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 621340. Size Standard: $7Mil All interested bidders shall submit quotations electronically by email to dan.nardi@med.navy.mil. E-mail submissions are limited to 2MB. The submitter should confirm receipt of email submissions. Site visit to be attended on Friday, 24 July 2009 @ 10:00am PST. Statement of Work 1.Scope: The contractor is required to provide all management, supervision, labor, material, and tools to remove and dispose of existing audio test chambers and to provide and install new audio test chambers per the contract. 2.Location: The Occupational Audiology Department is located at the Branch Medical Health Clinic, Marine Corps Recruit Depot, Bldg #596, Rm #M-7, 35000 Guadalcanal St., San Diego, CA 92140. 3.Room Measurements: The contractor is responsible for the measurement of all rooms where work will be performed under this contract to ensure that the audio test chambers installed by the contractor will fit properly and are workable. Contractors will be provided an opportunity to attend a site visit as listed below. 4.Site Visit: A site visit will be conducted with a date/time to be arranged by the Contracting Office. It's vital that contractor's attend the site visit to view the work area and perform tasks required, such as measuring the spaces, to accurately assess the Government's requirement for work under this contract. 5.Government Point of Contact: xxxxxxxx, Supervisor for the Occupational Audiology Department, can be contacted at 619-xxx-xxxx or email:xxxxxxxx@med.navy.mil. 6. Removal of Existing Audio Test Chambers: The contractor is required to disassemble, remove and transport the existing audio test chambers as described in this contract and dispose of the audio test chambers. If the contractor is in-doubt regarding removing a piece of equipment, they should contact the Government representative (Mr. Mark Hayman) for direction before proceeding. 7 Install New Audio Test Chambers: The contractor is required to provide and install two IAC Model 40m-10 Multi Test Chambers as described in this contract. This provides test positions for 10 patients in each chamber. The outside dimensions: (l) 6'10" x (w) 11'6" x (h) 7'10". This will include audiometric jack connector facility, built in lighting and standard "fan" ventilation. The color and final configuration to be advised at the time of order. The contractor is also required to provide and install one IAC Model 40a-3 Test Chamber, inside dimensions(l) 3'6"x(w)6'8"x(h)6'6" and outside dimensions (l)4'2"x(w)7'4"x(h)7'9". Standard lighting, ventilation and AC outlets. Color and configuration to be determined at time of order.
