Description
Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. The Richard L. Roudebuch VA Medical Center (RLR VAMC), Indianapolis, IN, is conducting a market survey for qualified firms capable of providing the required infectious waste services outlined below. Applicable NAICS Code is 562211 size standard $12.5 million. Responses to this notice must be submitted in writing (email or fax is acceptable) and received not later than July 17, 2009. No telephone inquiries will be accepted. Description of work to be performed includes: Contractor shall provide all labor, supervision for delivery of clean containers and pick-up of regulated medical waste. Contractor shall also supply materials, containers and equipment for packaging, removal and disposal of regulated medical waste at the Richard L. Roudebush VA Medical Center, 1481 W. 10th Street, Indianapolis, IN 46202. Contractor shall provide necessary personnel to unload clean containers for packaging. Contractor shall furnish the necessary reusable containers used for transporting the waste to an EPA approved disposal site. All shipping containers shall have approximately 28-gallon capacity or greater. The disposal of items shall be in accordance with all applicable Federal, State, and Local regulations and laws. Methods of disposal shall be by means of autoclaving or burning the waste in an EPA approved incinerator at a minimum temperature of 1832 degrees Fahrenheit. Contractor shall provide a 48' trailer to store packaged waste for transporting to disposal site. A pick-up schedule shall be established with the RLR VAMC for once a week or an on-call basis. Contractor shall warrant and represent that all personnel assigned to the premises under this contract are their employees and have been trained in the performance of the duties set forth in this contract, in accordance with Federal, State and Local Regulations. The Contractor agrees to comply with the policies of the Medical Center to include compliance with the standards of the JCAHO. Contractor shall comply with federal, state and local waste handling requirements relating to infectious/contaminated medical waste when hauling such materials from VA property to appropriate collection site of waste hauler. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, HubZone, 8(a), small disadvantaged business, small business or large business) relative to NAICS 562211 (size standard of $12.5 million per annum).Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, examples of similar facilities to which you have provided similar services to, DUNS number, address, point of contact and the size of your business pursuant to the following questions: (1) Is your business large or small? (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified HubZone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business? Responses to this notice must be submitted in writing (email or fax) and received no later than July 17, 2009. No telephone inquiries will be accepted. A solicitation is not currently available. If a solicitation is issued, it will be announced on the Federal Business Opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
