- Notice of Intent to Sole Source - Mentor/Byron Medical Lysonix 3000 Ultrasonic System Solicitation Number: VA-250-09-RQ-0259 The Department of Veterans Affairs, Louis Stokes VA Medical Center intends to solicit and negotiate with only one source under the authority of FAR 6.302-1, Only One Responsible Source, based on proprietary issues and compatibility requirements. The VA intends to solicit and negotiate with Mentor/Byron Medical, 201 Mentor Drive Suite 200, Santa Barbara, CA 93111 for a Lysonix 3000 Ultrasonic System with PulseSelect and FineLine Micro handpieces. This notice of intent is not a request for competitive proposals. However, any responsible source who believes it is capable of meeting the requirement may submit a capability statement, proposal, or quotation, which shall be considered by the agency, only if received by the closing date and time of this notice. A determination not to compete the proposed requirement based upon the responses to this notice is solely within the discretion of the government. Responses to this notice are due on or before 17 July 2009, 11:00 AM EST by email to adrian.cadena@va.gov and marva.jordan@va.gov.
