- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The request for quote number is N684708063FADA01. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; FAR 52.212-2 Evaluation - Commercial Items; FAR 52.212-3 Offeror Representations and Certifications --Commercial Items; and FAR 52.212-4 Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items; additional FAR clauses cited in the clause are: 52.222-3, Convict Labor; 52.222-26, Equal Opportunity; 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports, NAICS Code for this requirement is 325413. The intention is to award a Firm-Fixed Price contract. The U.S. Naval Hospital, Okinawa, Japan intends to negotiate a sole-source contract with AcroMetrix for the following items. 1. VeriSure Pro WNV Control (Positive & Negative); # 96-0700; 9 BX Reagents MUST be delivered on OCTOBER,FEBRUARY,and JUNE the expiration date no less than 1 year. Three(3) per shipment. 2. VeriSure Pro Control HCV/HIV/NEG; # 96-3700; 9 BX Reagents MUST be delivered on OCTOBER,FEBRUARY,and JUNE the expiration date no less than 1 year. Three(3) per shipment. 3. VeriSure Pro HBV Positive Control; # 96-3603; 27 BX Reagents MUST be delivered on OCTOBER,FEBRUARY,and JUNE the expiration date no less than 1 year. Nine(9) per shipment. However, any responsible source that can meet the requirements may submit the written notification to the Point of Contact (POC). Supporting documents must be furnished in sufficient detail, to demonstrate the offeror?fs capability to provide and ability to comply with the requirements. Offerors responding to this notice must be FDA approved for the manufacture, sale and distribution of these products. Offerors must be registered in Central Contractor Registration. [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award] Registration is available via the Internet at www.ccr.gov. The place of delivery will be directly to U.S. Naval Hospital, Okinawa, Japan, BLDG. 6000, Camp Lester, Okinawa, Japan 904-0103, via Naha Airport. The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Technical capability of the item quoted to meet the Government requirement (including submission of descriptive literature); (2) Price. Quotes must be submitted to the POC no later than July 24, 2008; 10AM; Japan standard time. Quotes shall be submitted via e-mail in Word, Excel, or PDF format to the POC, Denzil D. Lawrence, at Denzil.Lawrence@med.navy.mil or faxed to 011-81-98-893-5754. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, U.S. Naval Hospital, Okinawa, Japan will proceed with the sole source procurement.
