Description
THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a an Indefinite Delivery Indefinite Quantity (IDIQ) contract. The Government will use responses to this notice to make to appropriate acquisition decision. The contractor will be required to perform radiological procedures/services, for the Cheyenne Veteran Affairs (VA) Medical Center, Cheyenne Wyoming. These radiological procedures/services include but are not limited to the following: CT, Skull, Chest, Abdomen, Spine, Bone and Joint, GI, BE, GU, IVP, GB, Cholangiogram/T-Tube, tomograms, venograms, myelograms, Arthrograms, Chest Fluoro, Mammograms, Ultrasounds, Biopsies, Non-invasive vascular studies and Peripheral/Doppler Ultrasounds. The contractor shall provide on-site procedures, such as GI's BE's or biopsies at least twice a week. The contractor shall have the capability to respond to emergencies by phone within 15 minutes and be on site within 1 hour on any day of the year. The contractor shall also provide clinical direction and be available to attend functions that require the contractor's input. The contractor shall be available to the VA for consultation, film interpretation, emergency procedures, 24 hours a day, 7 days a week, 52 weeks a year. Subcontracting to perform the requirements of this contract will not be allowed without the expressed written permission of the Contracting Officer. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contract Specialist Matt Ake no later than 3:00 p.m. (Mountain Time), Monday July 13, 2009. Solicitation VA-259-09-RP-0298- CHEYENNE RADIOLOGY SERVICE has been assigned and should be referenced on all correspondence regarding this announcement. NAICS Code 621512 is applicable to this acquisition, the size standard for a firm to be considered has an Emerging Small Business is no greater than 50 percent of NAICS Code with a Business Size Standard of $13.5M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
