Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (III) The provisions and clauses incorporated herein are those in effect through Federal Acquisition Circular 2005-23, effective December 26, 2007. This requirement is being synopsized as full and open competition under standard industrial classification code 8228, NAICS Code 541690, size standard is $7.0 million. The resulting contract will be a Firm Fixed Price type Contract under FAR Part 12 - Acquisition of Commercial Items. COMMERICAL ITEM DESCRIPTION: The Department of Veterans Affairs Medical Center/Employee Education Resource Center, St. Louis, MO proposes to enter into a firm-fixed contract for Safety Training which requires 21 instances of 2 courses to bring staff to the level of OSHA recognized competent person in the areas of Scaffolding and Trenching and Excavations.Proposals are due Thursday July 16, 2009 by 3:00 pm CST to Linda Eley, linda.eley@va.gov, or fax to (314)894-6436. Questions can also be permitted no later than Tuesday July 14, 2009 @ 1:00 pm CST. All questions must be addressed to the Contracting Officer/Specialist only. Any other official may disqualify the offeror from submission of a quote. PERFORMANCE STATEMENT OF WORK SECTION 1.0 GENERAL SCOPE Safety Training (10NS and EES) require 21 instances of 2 courses to bring staff to the level of OSHA recognized competent person in the areas of Scaffolding and Trenching and Excavations. These courses are required to be delivered on regional sites located at 21 locations to be negotiated with the vendor and EES. A proposed course outline is attached to this statement of work. The course must be compliant with OSHA 29 CFR 1910 and 29 CFR 1926. A list of required courses are: Course 1: Scaffolding Course 2: Trenching and Excavation The contractor shall provide any and all labor, supplies (including but not limited to copies of curriculum in a three ring binder), and any other materials necessary to present a two day ( 16 class hours) course titled Trenching and Scaffolding Competent Person Training. The course will be taught at various VAMC locations. SECTION 2.0 CONTRACTING OFFICER TECHNICAL REPRESENTATIVE TERMMEANING Contracting Officer Technical Representative (COTR) John Scott Adams Phone: 501-257-4204 FAX: 501-257-4190This is the person who works with the contractor to make sure that the contract is fulfilled to the satisfaction of the VA. SECTION 3.0 GOVERNMENT FURNISHED PROPERTY, MATERIALS, SERVICES GOVT. SECTION 4.0 CONTRACTOR FURNISHED CRITICAL ITEMS / SERVICES ITEM #QUANTITYDESCRIPTION Course materialsOne per student Copies of all instructional materials to include copies of presentations, copies of handouts, and copies of any other instructional materials issued with this course. These materials shall be freely copyable. SECTION 5.0 PERFORMANCE WORK STATEMENT TASKS AND CRITICAL SUBTASKS 5.1. - Course Design Contractor shall provide two eight (8) hour courses at each agreed location. Courses outlined in Section 5.1.1 and 5.1.2 shall be held on concurrent days with eight (8) hours of instruction per course totaling sixteen (16) hours of instruction for both courses. 5.1.1. - Course Design Scaffolding Course one (1) shall be based on the standards for scaffold hazards based on 29 CFR 1926 focusing specifically on 1926.454. The course shall have the following objectives Students will understand: "The nature of any electrical hazards, fall hazards and falling object hazards in the work area (1926.454(a)(1) ) "The nature of any electrical hazards, fall hazards and falling object hazards in the work area; (1926.454(a)(2)) "The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used; 1926.454(a)(3) "The proper use of the scaffold, and the proper handling of materials on the scaffold; 1926.454(a)(4) "The maximum intended load and the load-carrying capacities of the scaffolds used; 1926.454(a)(5) "Any other pertinent requirements of this subpart. 1926.424 (a), 1926.424 (b) "The nature of scaffold hazards; 1926.454(b)(1) "The correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question; 1926.454(b)(2) "The design criteria, maximum intended load-carrying capacity and intended use of the scaffold; 1926.454(b)(3) "How to recognize situations when the employer has reason to believe that an employee lacks the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds; 1926.454(c)(1) "Understand, recognize and know what corrective actions to take when presented with a reason to believe that an employee lacks the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer shall retrain each such employee so that the requisite proficiency is regained. 1926.454(c) "Understand, recognize and know what corrective actions to take when presented with a worksite hazard about which an employee has not been previously trained; 1926.454(c)(1) "Understand, recognize, and know what corrective actions to take when presented with changes in the types of scaffolds, fall protection, falling object protection, or other equipment present a hazard about which an employee has not been previously trained ; 1926.454(c)(2) "Understand, recognize, and know what corrective actions to take when presented with inadequacies in an affected employee's work involving scaffolds indicate that the employee has not retained the requisite proficiency. 1926.454(c)(3) 5.1.2. - Course Design Trenching Course two (2) shall be based on the standards for scaffold hazards based on 29 CFR 1926 focusing specifically on 1926 Subpart P, Excavations ; 1926 Subpart S, Underground construction, caissons, cofferdams, and compressed air, and 1926 Subpart V, Power transmission and distribution Sloping Aluminum Hydraulic Shores Trench Boxes Soil Analysis Techniques and Requirements OSHA Compliance Activities for Excavations "Students will understand and identify common excavation hazards 1926.651, Specific excavation requirements "Students will be able to replicate conditions following the requirements of 1926 Subpart P for General Requirements for Excavations "Students will understand and be able to follow the 1926.652, Requirements for protective systems for trenching and excavations "Students will understand basic requirements and definitions of soil mechanics including "Tension cracks "Sliding/sluffing "Toppling "Subsidence and bulging "Heaving or squeezing "Boiling "Students will be able to determine and classify soil types, differentiating between "Stable Rock "Type A Soils "Type B Soils "Type C Soils "Layered Geological Strata "Students will understand and be able to replicate methods for evaluating soil type and operate relevant test equipment to include: "Pocket Penetrometer "Shearvane or Torvane "Thumb Penetration Tests "Dry Strength Tests "Plasticity or Wet Strength Tests "Visual Tests "Appendix A, Soil classification "Students will understand and be able to describe significant shoring types including "Appendix C, Timber shoring for trenches "Appendix D, Aluminum hydraulic shoring for trenches "Appendix E, Alternatives to timber shoring "Appendix F, Selection of protective systems "Students will understand appropriate methods of operation and methods for safely isolating underground power lines consistent with 1926.956, underground lines 5.2 - Course Presentation Each instance of course will be presented over a period of 2.0 eight hour class days. The course will include didactic and experiential components. The specific presentation requirements include: 5.2.1. Scaffolding "Introduction "Definitions "Planking "Inspection "Rolling Scaffolds "Inspection of Rolling Scaffolds "Inspection of Scaffolding "Allowable spans and planking "Bracing "General Requirements "Health and Safety Considerations "Site Assessment Questions 5.2.2 Trenching, Shoring and Excavation "Introduction "Definitions "Overview "Determination of Soil Type "Test Equipment "Shoring Types "Shielding Types "Spoil "Sloping and Benching "Health and Safety Considerations "Site Assessment Questions 5.3 - Materials The contractor shall provide either electronic copies or a 3-ring binder with copies of all instructional materials utilized in this course. 5.4 - Records, Data & Reports 5.4.1 Lesson Plans Contractor shall provide lesson plans for the government project manager's approval prior to class presentations. 5.4.2 Handouts Contractor shall provide copy of handouts to the Project Manager a minimum of two weeks prior to the courses. 5.4.3 Quality Control Plan A written and performance based examination covering skills taught in both Section 5.1.1 and 5.1.2 will be administered on site. The examinations will cover material drawn from the current version of 29 CFR 1926.454 and 29 CFR 1926.651. All students who make a passing score (80%) on the test will receive a completion certificate and card for course content. Reports of all student status on the post-test will be submitted to the VA. In compliance with the contract clause entitled "Inspection of Services - Fixed Price," FAR 52.246-4 the Contractor shall provide a Quality Control Plan to the Contracting Officer for acceptance not later than five days after award of contract. The Contracting Officer will notify the Contractor of acceptance or required modification to the plan before the contract start date. The Contractor shall make appropriate modifications and obtain acceptance of the plan by the VA before the contract start date. 5.5 - Course Completion At the end of the course, the VA will have access to course evaluations completed by the participants. The vendor will provide a written and performance based examination covering skills taught in both Section 5.1.1 and 5.1.2 will be administered on site. The vendor will provide (a) a copy of any examination given students and (b) completion records on each student enrolled in the course by VA staff. The examinations will cover material drawn from the current version of 29 CFR 1926.454 and 29 CFR 1926.651. All students who make a passing score (80%) on the test will receive a completion certificate and card for course content. Reports of all student status on the post-test will be submitted to the VA. 5.6 - Evaluation Factors Several evaluation factors shall be considered when awarding this contract. These include the ability of the Offeror to achieve the above objectives and satisfactorily complete the tasks contained within this Statement of Work. In addition, the offeror's past technical performance related to Trenching, and Scaffolding according to OSHA standards, personnel qualifications, experience and price will be considered. Contract award shall be made to the Offeror whose proposal represents the best overall value to the government. The offeror must provide documentation to support these qualifications as described in the technical review factors for this solicitation. The Government will award one contract resulting from this solicitation to the responsible Offeror, whose offer conforming to the solicitation, will be most advantageous and providing the best value to the Government, price and other factors considered. It is the intent of the Government to make an award without negotiations. Offerors are encouraged to submit their best price. Submit concise quotations. The following factors shall be used to evaluate offers: Priority 1.Work Sample. The selected offeror must have knowledge of OSHA Guidelines regarding Trenching and Scaffolding, recognized methods of instructional design and incorporate effective methods of hands on instruction including field work and miniaturized sample equipment. a. Submit one document (MS Word) containing lesson plans and needed equipment listings, methods for evaluating knowledge, and lesson objectives. NOTE: work sample must demonstrate: comprehensive knowledge of OSHA training requirements for working with and training around Trenching and Excavation requirements and Scaffolding requirements to lead to competent person status. b. Work sample(s) shall be submitted via MS Word or HTML Document included in bid submission. Note: Work samples submitted via DVD or CD-ROM will not be reviewed or returned. Priority 2. Price. The prices shall cover all costs related to delivering one job as described in accordance with the specifications. No other pricing information will be required. The VA has extensive experience developing face to face courses and historical funds expended. The price in comparison to the taxonomy and complexity of the work is the basis for this factor. Priority 3.Capability to Perform the Work. a. Provide the names of individuals that shall be working on this project who have technical experience in developing OSHA Trenching and Scaffolding courses. Include a resume for each person who will be working on the project. b. If the Contractor plans to add additional personnel to the project, after award of the contract, the Government reserves the right to either accept or reject any additions to the people working on this project. Priority 4.Past Performance a. Furnish the title of your last three face to face course projects. Include the name of the Government Agency or Company contracting the work, the name and phone number of the Contracting Officer (if a Government project) or the name and phone number of the Government Project Manager administering the project. The Government may or may not contact any or all references provided. 5.7.7 5.7 Performance Requirements, Delivery Schedule and Performance Standards: Task 1: Contract Kick-off Meeting. The selected Contractor shall attend a kick-off meeting with the VA Project Team Leaders and Contracting Officer Technical Representative within 5 business days after award of the contract and shall take minutes of the meeting. The meeting may be conducted by telephone or in person. The offeror shall submit a separate travel estimate for this meeting with their bid proposal. If it is determined that the meeting will be conducted via teleconference, then no funds would be allocated for travel. If the kick off meeting is conducted face to face, the meeting will be conducted at the Employee Education Resource Center. The address is: Department of Veterans Affairs Employee Education Resource Center 2200 Ft Roots Drive, Bldg 34 North Little Rock, AR 72116 During the kick-off meeting, VA staff members attending and the selected contractor will: "Review the Statement of Work (SOW) and discuss the detailed requirements of all deliverables, EES IT infrastructure, and VA desktop/laptop platform "Review and discuss the effective learning strategies to be considered for incorporation into the face to face courses on Scaffolding and Trenching and Excavation. "Review and discuss technical details including OSHA Compliance. "Review the Project Plan Document Deliverable 1: The kick-off meeting and minutes. Minutes to be provided two days after Kick Off Meeting. Task 2: Project Plan The Contractor shall provide a project plan document to the VA Staff members attending the kick off meeting. The project plan document will contain the following elements in the following order: Executive Summary: Include an outline of the design solution and time and price to complete the work. (Maximum of two pages) Understanding of the Scope: Provide a concise description of the challenge you believe VA is facing in regards to this project. Description of Instructional Solution: Provide detailed information on the solution to our challenge in terms of instructional elements. Clearly outline an instructional methods and strategies. Design and Development: Provide samples of the lesson plans to be utilized for all elements of each course. Also, provide descriptions of learning aids, materials, and needs to be provided at each VISN site. The design and development should be comprehensive for each unit, module, lesson, and topic level of the course. Description of Technical Solution: Provide detailed information on providing a solution to our challenge in terms of technical requirements and considerations. Project Management and Schedule: Describe your approach. Include a work plan outlining resources (yours and ours), phases, timelines, and deliverables. The project plan document shall be delivered as an attached Microsoft Word or Adobe PDF document via e-mail to the COTR, the Project Manager, and to the Contracting Officer. NOTE: E-mail message delivery limitation. E-mail messages with a file size, including attachment(s), over 3 MB cannot be received. Deliverable 2: Project Plan to be delivered five working days after award of contract.. Task 3: Scheduling of travel and class locations. The Contractor shall provide a negotiated schedule working with the COTR and field staff determining times and locations of the 21 (twenty-one) field delivery locations of the course delivery. The purpose of the schedule is to ensure adequate field resources are acquired to ensure effective delivery of the course and also to determine if there are any instructional system design issues that must be clarified prior for the development of the Trenching and Scaffolding courses. Deliverable 3: Schedule of travel and locations: Due twenty (20) working days after award of contract Task 4: The Contractor will deliver lesson plans and related handouts for the Trenching and Scaffolding courses to the COTR. Finalized lesson plans and associated handouts will be sent to the COTR within 45 working days after award of the contract. NOTE: The VA will be providing copies of the documents to field locations. Adequate time must be provided to copy and distribute these documents Deliverable 4: Finalized lesson plans and associated handouts.. Task 5: The Contractor shall deliver a two-day (totaling 16 hours on instruction) version of the courses outlined in Section 5.2 at 21 VISN locations. The course delivery shall occur at 21 National VISN level sites. EES Standard Evaluations will be provided and returned to the COTR. Deliverable 5: The completed course evaluations and other related course documentation indicating student knowledge. Task 6: Lists of students and relevant evaluations of knowledge who completed the course. The Contractor shall forward a list of all students who successfully completed the course to include complete evaluation results of all learning evaluations. NOTE: Learning evaluations are to include ratings for rubrics detailing performance and finalized scores of knowledge gains during the course. Deliverable 5: Per course delivery; A)list of students who completed the course B)Relevant evaluation data C)This information may be forwarded via email in MS Word or Excel format. Task 7: Microsoft Word Copy of Web Course. The VA anticipates that these courses will be valid for ten years. Over that period, each course is reviewed annually for content modifications. The VA requires that when the Final Web Course is approved, that a MS Word Document shall also be developed for use by its content experts for future updates. Deliverable 7 - One MS Word Document for each course. Task 8: Archive and delivery of all computer files related to this project. The Contractor shall provide all project source files and programmer notes to the COTR in acceptable unencrypted file formats and on an acceptable archival medium DVD. Deliverable 8: The archive Task 9: Weekly Status Reports. The Contractor shall provide the Contracting Officer and the COTR with a weekly status report by email each Tuesday reflecting progress of the project plan of Task 2. Deliverable 9: Weekly Status Reports 6.0 Delivery Schedule. The delivery schedule is base on the date of award of contract to a selected offeror. Delivery Schedule: Deliverable 1: The kick-off meeting and minutes Minutes to be provided two days after kick off meeting Deliverable 2: The Project Plan Five working days after award of contract. Deliverable 3: Content Review:PSM I TBD PSM II TBD Deliverable 4: The Alpha version of the Course 1-7 Alpha version of PSM I and II. Deliverable 8: The archive Five working days after acceptance of VA Final Web course Deliverable 9: Weekly Status Reports Each Tuesday to COTR Final Delivery Date: This Contract Final Delivery Date will not extend past September 30, 2009. SECTION 6.0 GOVERNMENT PUBLICATIONS/INDUSTRY STANDARDS/STATE STANDARDS. GOVERNMENT STANDARDS/ TECHNICAL EXHIBITS NUMBERTITLEAGENCY 29 CFR 1926Safety and Health Regulations for Construction Occupational Safety and Health Administration SECTION 7.0 RIGHTS IN DATA AND FINAL DELIVERABLE FORMAT: The Department of Veterans Affairs is responsible for maintaining and modifying the final deliverables over time. All elements contained within the production version of the final deliverables will need to be modifiable by VA professional staff with knowledge and skills to create documents, build web sites, graphics and flash streaming video and animation. This requires the offeror upon VA acceptance of the final web course product to package all source files utilized to develop the final content storyboard, graphics - photo shop files, flash source and executable files and the final production version of the Web source files to the Department of Veterans Affairs Contracting Officer Technical Representative on CD/DVD. Note files may not be delivered with password protect files which prevent VA professionals from modifying files at a future date. The vendor owns the copyright for handouts, but grants DVA a license to copy materials as determined necessary. The vendor agrees to keep all employee personnel information gained as a result of this course confidential, only reportable to non-VA sources as required by federal, state or other governmental agencies. SECTION 508 - ELECTRONIC AND INFORMATION TECHNOLOGY (EIT) STANDARDS The Section 508 standards established by the Architectural and Transportation Barriers Compliance Board (Access Board) are incorporated into, and made part of all VA contracts, solicitations and purchase orders developed to procure Electronic and Information Technology (EIT). These standards are found in their entirety at http//www.section508.gov. A printed copy of the standards will be supplied upon request. The contractor shall comply with the technical standards at 36 CFR 1194.22 Web-based Intranet and Internet Information and Applications, 1194.24, Video and Multimedia products, and 1194.31, Functional Performance Criteria in performing this order.
