Description: Statement of Work (Equipment List included) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z368914A001; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-33, effective 15 June 2009, Defense DCN 20090115, and AFAC 2009-0318 effective Mar 18, 2009. This acquisition will be a Total Small Business Set Aside; the North American Industry Classification System (NAICS) code is 713940. The business size standard is $7.0M. The Federal Supply Class (FSC) is G003. The Standard Industrial Classification (SIC) is 7991. This request for quotation consists of the following: CLIN 0001: Preventative Maintenance on Gym Equipment, Contractor shall provide labor, parts, materials and any necessary items needed to provide preventive maintenance and on call services for all strength and cardio equipment in bldg 434, Travis AFB Fitness Center, Travis AFB CA. Service will include, but not limited to inspections of parts for wear, adjustment impending breakdown, adjustments for wear, worn or damaged parts, cleaning, lubrication, and protection from deterioration by corrosion to ensure safe and dependable operational status of equipment at all times. Period of Preformance: 1 Oct 2009 through 30 Sep 2010. See attached documents: Statement of Work, Equipment List, and Wage Determination.
