- List of Required Products for Quote This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included in this notice. A quote is being requested and a written solicitation will not be issued. Requirement number 770-9-0076-02 is issued as a Request for Quotation. The Centers for Medicare and Medicaid Services (CMS), has a requirement for a contractor to provide additional MapInfo licenses and professional services for use in developing GIS/Mapping implementation for Part B Analytics and MIIR applications. The attached page lists the details of the required items. The period of performance will be June 30, 2009 to June 29, 2010. This will be a firm fixed price procurement using simplified acquisition procedures. The North American Industry Classification System Code (NAIC) is 511210. This Request for Quotation (RFQ) is being set aside for small business concerns. A purchase order will be awarded within 3 days after the response due date. Your response must be mailed in paper form to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd, Mailstop: C2-21-15, Baltimore, MD 21244 or electronically in MS Word format to phillip.harrell@cms.hhs.gov no later than 11:00am on June 25, 2009. For the purpose of establishing timely receipt of a quotation, the time will be established based upon the hard copy submission. Responses shall be no more than 5 pages and should include the following: 1) Contractors brief capabilities overview; 2) Past performance information (relevant experience to perform the required services); and 3) Price of the deliverables. The Government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price. All contractors are hereby advised that contractors shall be registered in the Central Contractor Registry (CCR). Please register with the CCR at the following website: http://www.ccr.gov.
