- Description
- The 23rd Contracting Squadron/LGCA2 plans to establish a Blanket Purchase Agreement for Medical Waste Disposal. The BPA will be for a period not to exceed 5 years. Please see attached Statement of Work for details on the services requested. Please submit a price list by fax to 229-257-3547 or by e-mail to ellen.feazell@moody.af.mil,. Price lists will be set for 1 year, after this period, prices may be updated. Place of performance: 23rd Medical Group, 3280 Mitchell Blvd, Moody AFB, GA 31699 1. Description of Service: Contractor shall provide all labor, tools, supplies, and equipment to pick-up and dispose of medical waste generated by the23rd Medical Group. Service shall be provided once per month at minimum. The contractor shall be responsible for the following: Provide appropriate collection, storage, and shipping containers. Preferred container is a 4.3 cubic foot cardboard box with appropriate regulatory markings. Pick-up times are 0730 - 1630, Mon - Fri, excluding federal holidays. Contractor shall provide a copy of the waste manifest along with the certificate of destruction. Ensure all containers are appropriately marked as a biohazard per EPA requirements. Collect and transport medical waste via sealed truck to EPA approved disposal site designated by the contractor. Ensure waste is destroyed IAW local, state, and federal regulations. Sign the manifest prior to departure from the pick-up site. One copy of the manifest will be provided to the Facility Manager or his designee. Manifest will contain the following information at minimum: Generators name and mailing address Transporters name and mailing address Phone numbers of the generator and contractor Destination Tracking number Date and signature of transporter Call customer 7 days prior to confirm pick-up dates. 2.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES: Government will provide secure storage space for containers.
