- Statement of Work-Carpet Squares Contracting Office Address Department of Homeland Security, Federal Emergency Management Agency, 1111 Broadway, Suite 1200, Oakland, CALIFORNIA, 94607-4052, UNITED STATES Description THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. ***This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18.*** *** The associated North American Industrial Classification System (NAICS) code for this procurement is 238330 with a small business size standard of $14.0M.*** *** The Department of Homeland Security/Federal Emergency Management Agency Region IX is seeking to identify companies that can provide services for installation of the 11th Floor DHS/FEMA Region IX Carpet Squares*** ***All interested Contractors shall provide a quote for the following: STATEMENT OF WORK IS ATTACHED FOR YOUR REVIEW PLEASE CONTACT THE CONTRACTING OFFICER BY EMAIL AT AMEKA.ALI@DHS.GOV TO SCHEDULE A SITE VISIT. SITE VISITS WILL BE CONDUCTED JUNE 8-16, 2009. Award shall be made to the quoter whose quotation offers the best value to the government. ***The award is intended to be a firm-fixed price (FFP) order.*** All responsible sources may submit an offer which shall be considered by the agency. Offers for the items(s) described above are due by 2:30 pm PST, June 29, 2009, FEMA Region IX, Attn: Ameka Ali, 1111 Broadway, Suite 1200, Oakland, CA 94607-4052 and must include solicitation number, FOB destination to this Center, proposed delivery schedule, discount/payment terms, warranty duration (if applicable), taxpayer identification number (TIN), identification of any special commercial terms, and be signed by an authorized company representative. Offerors shall provide the information required by FAR 52.212-1 (JUNE 2008), Instructions to Offerors-Commercial Items, which is incorporated by reference. Addenda to FAR 52.212-1 are as follows: N/A The full text of FAR provisions or clauses may be accessed electronically at http://acquisition.gov/comp/far/index.html. The following solicitation provisions apply to this acquisition: •FAR 52.212-1, "Instructions to Offerors-Commercial Items" •FAR 52.212-3, "Offerors Representations and Certifications-Commercial Items" Offerors must complete annual representations and certifications on-line at http://orca.bpn.gov in accordance with FAR 52.212-3, "Offerors Representations and Certifications-Commercial Items." If paragraph (j) of the provision is applicable, a written submission is required. The following contract clauses apply to this acquisition: •FAR 52.212-4, "Contract Terms and Conditions-Commercial Items" •FAR 52.212-5, "Contract Terms and Conditions Required to Implement Statutes or Executive Orders" The following subparagraphs of FAR 52.212-5 are applicable: All quoters shall submit the following: two (2) quotes by email at Ameka.Ali@dhs.gov. This is an open-market combined synopsis/solicitation for equipment as defined herein. The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, ALL quotes shall include a statement regarding the terms and conditions herein as follows: The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition. Submission shall be received not later than 2:30 PM PST at Ameka.Ali@dhs.gov or 1111 Broadway, Suite 1200, Oakland, CA 94607-4052. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). E-mailed quotes will be accepted.
