Description
Wage Determination under the Service Contract Act VPAT Instructions Non-Disclosure Agreement Government QASP Sample NIH Contractor Performance Report SF3881 ACH Vendor Billing Instructions WiseWoman Solicitation This Centers for Disease Control and Prevention (CDC) solicitation is a small business set-aside concern for the Data Management and Technical Assistance for the Division for Heart Disease and Stroke Prevention - WISEWOMAN Program. The primary purpose of this contract is to provide system support and technical assistance to facilitate collection and analysis of standardized data elements. These data will be used to evaluate accomplishments and to provide extensive technical assistance and consultation that promote program improvement and efficiency to CDC's WISEWOMAN Program and its grantees. The contractor shall provide all labor, supplies, materials, supervision and perform all services required to: (1) Prepare, maintain and enhance data systems and procedures to collect and report program data to CDC and grantees; (2) Perform data analysis and reporting; (3) Develop and host the web site for data input; (4) Provide technical assistance to CDC and grantees; (5) Provide analytical support to special projects. All deliverables to be disseminated to the programs or the public will comply with Section 508 (29 U.S.C. 794d) standards and requirements of the Rehabilitation Act. Contractors shall E-mail all questions to this solicitation to Darlene Seminko at DSeminko@cdc.gov no later than June 17, 2009. No questions will be accepted by telephone. Answers to questions shall be posted on Fedbizopps by June 19, 2009. It is your responsibility to keep up with additional postings regarding this solicitation. All proposals are due on June 8, 2009 by 5:00 P.M. EST.
