- Description
- (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.(ii) The solicitation number is VA-246-09-RQ-0163 and is issued as request for quotation (RFQ). (iii) This solicitation document incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. (iv) This requirement is not set-aside but is advertised Full and Open Competition. The Small Business Competitiveness Demonstration Program does not apply. (v) This requirement is for One (1) each AngioDynamic Delta 15 Surgical Diode Laser for endovenous laser treatments (EVLT), training video for physicians and patients; 3 pairs of safety glasses; manuals, 2 keys; foot peal, 2 warning signs; 2 remote interlock connectors; 4 spare fuses; 1 test fiber; 64 MB MMC card; power cord; and installation. (vi) Unit has fiber recognition for immediate activation of procedure presets, MultiMedia Card stores data from up to 100 treatments; real-time on screen display of energy; automated tracking of procedure and fiber records; patient data management software and USB-MMC Card Reader/Writer for data transfer to PC; key fob/MMC Card holder for storage of card and laser keys; three (3) year standard warranty and loaner program. (vii) Delivery date is 30 days after receipt of order; to the VA Medical Center Warehouse, 508 Fulton Street, Durham, NC; FOB - Destination. (viii) The following provisions apply to this solicitation: 52.212-1, Instructions to Offerors -- Commercial, 52.212-2 Evaluation -- Commercial Items based on brand, lowest price. If bidding on other than specified item, must include product specification sheet. (ix) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with the offer. (x) The provision 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
