- SF 33 to be completed and submitted as part of proposal. The Uniformed Services University of the Health Sciences (USUHS), located in Bethesda, MD on the National Naval Medical campus has a requirement for consulting services to examine and evaluate USUHS application process for reaccreditation by the Accreditation Council for Continuing Medical Education (ACCME). This requirement includes, but is not limited to, conducting a process evaluation of self study and preparing a comprehensive report of at least 300 pages demonstrating full or exemplary compliance with each of the Areas and Elements dictated by the ACCME and Standards for Commercial Support. This is a request for proposals (RFP) issued in accordance with FAR Part 13.5-Test Program for Certain Commercial Items. The Government contemplates award of a Firm Fixed Price contract with one base period/phase and two option periods/phases resulting from this solicitation. This procurement is set aside for small businesses only. The applicable NAICS code is 541611- Administrative Management and General Management Consulting Services with a size standard of $7.0 million. Offerors must be registered in the Central Contractor Registration (www.ccr.gov) as a small business under NAICS code 541611 by the close date of the RFP. Those offerors not registered in CCR as a small business under NAICS code 541611 will be considered nonconforming to the requirement and their quote will not be considered. The contractor shall provide pricing as specified in the attached solicitation (SF 33). Further instruction concerning the consulting services to examine and evaluate USUHS application process for reaccreditation to the Accreditation Council for Continuing Medical Education (ACCME) is found in the attached statement of work. Vendors must submit the completed SF 33 and a projected and detailed travel expense breakdown as part of their proposal. Interested offerors must also provide DUNS and tax identification numbers. USUHS intends to award without discussions, but reserves the right to hold discussions if necessary. Please view the attached solicitation and past performance survey for additional information. All questions regarding this requirement must be submitted in writing by 12:00 p.m. (EST) on June 4, 2009 to ggermana@usuhs.mil. Questions will not be answered over the phone, but will be answered in an amendment posted on or around June 8, 2009. Questions submitted after June 4, 2009 at 12:00 p.m. (EST) will not be answered. It is the contractor's responsibility to monitor FedBizOpps for any amendments to this solicitation. Proposals shall be accepted by email only. No other methods of submission will be accepted.
